Posted on Tue, 14 Apr 2015
Question: Hi Doctor
I have done unprotected sex on 14th XXXXXXX 2015 and 16th Feb, 2015 and on 2nd March,2015 I have done HIV 1 RNA Qualitative test (sample type EDTA Plasma Blood) and HIV 2 real time PCR (sample type EDTA Plasma Blood). Both the results are negative and on 15th March, 2015 I did an HIV 1 and 2 + p.24 Ag (method CMIA) which was also negative.
Now I am getting married on 9th April, 2015. Can I have sex with her or do I need to wait. What kind of tests I need to do now.Any tests after 3/6 months. I am really worried, kindly advise me. Thank you

Brief Answer:
You are HIV negative

Detailed Answer:
Hi Mr XXXXXXX

Welcome to health care magic.

I have gone through the details of your uploaded reports and your query and I understand your concern regarding your HIV status.

I want to assure you that you can consider yourself HIV negative based on the reports you have uploaded on our link.

The tests you have undergone are the tests of choice to detect HIV during early exposure.

HIV can be detectable in blood sample 4-11 days after exposure by PCR and p24 antigen testing.

Both your test reports are negative for HIV 1 and 2.

You are HIV negative.

Quit worrying about these past episodes.

No further testing required.

Tip for the future:
Any sexual intercourse (outside of mutually monogamous relationship) should be protected by a condom.
Do not share unsterile needles or syringes.

Please get back if any more queries.

Wishing you all the good luck in your future

Thanks.

Hi Dr. XXXXXXX

I could see from the internet that some medical journals and doctors suggest for confirmation Hiv tests in three minths and six minths.

Brief Answer:
Confirmation tests done to rule out window period for antibody testing

Detailed Answer:
Hi Mr. XXXXXXX

Welcome back.

I understand your concern regarding confirmation of your HIV status.

Let me explain few points about HIV so that you can understand the testing strategies used for HIV better.

HIV is a RNA virus which enters into the body through small abrasions caused by intercourse in the membranes of the genital organs.

Once it enters, it is taken up by cells called macrophages which carry this virus to local lymph nodes. Here the virus multiplies in the cells of the lymph node and gets spilled over in the blood.

This takes about 4-11 days.

So there is no test which can detect HIV within 4 days of exposure.

After 11 days, the best test to detect HIV is PCR and p24 antigen.

The body tries to fight against HIV by producing antibodies, but these antibodies are detectable only after 3 weeks after exposure.

Some persons take longer to develop antibodies which can be up to 3 months.

This period is called window period. Here the person is HIV positive but tests negative for HIV antibodies.

The test kits commonly used for HIV screening and testing detect antibodies against HIV.

If such test is done and the result is negative, then a confirmation test is done after 3 months to rule out window period.

Confirmation tests include PCR and western blot tests.

Now coming to your case, you have undergone PCR testing and p 24 antigen testing which are considered the best tests to be done during early exposure.

Both your test results are negative, so your HIV status can be declared as negative.

No further testing is required.

Moreover, the net gives so much information about HIV which can be very misleading and distressing to the reader.

I suggest you trust your health care provider to give relevant information about HIV.

I hope I have helped reduce your worries.

Please do not hesitate to get back if any more queries.

Thanks.
