Brief Answer:
I would like to start antibiotic

Detailed Answer:
Hi

Thanks for asking on this platform

I have gone through the details and do understand your concern.
First I would request you to answer my few queries like
Is the skin reaction limited to genital area or it is there in other body parts also
What is the type of skin lesion(is there any discharging pus, fluid filled lesions, reddening or with scales
Is there any other family member who also has itchy lesions

Itchy lesion in genital area followed by fever in this age can be scabies, allergic rash or fungal rash with bacterial infection.
Skin lesions are best diagnosed by direct visualisation so if possible please attach an image so that I better able to help you out.
I won't get overly worried if child is otherwise active, playful and there is no breathing difficulty.
Here I would like to start the Phexin syrup which is an antibiotic. Applying a cream with antifungal and antibiotic will help(miconazole and gentamycin combination).
Continue applying calamine as it decreases itching by soothening the skin lesion.
Giving antiallergic like hydroxyzine/cetrizine will also help in relieving the itchiness.

I hope this helps.
Please let me know if you have any more questions.
Please get back with the replies for better assessment.
With regards

Brief Answer:
It's more allergic or scabies

Detailed Answer:
Thanks for follow up

I have seen the image. These are multiple fluid filled vesicles but in the attached image I did not find any sign of bacterial infection or pus To me it appears more like allergic in nature or scabies.
The point against scabies is the site of involvement. In scabies the involvement is widespread and usually involves hands also.
In allergy there is associated redness also.
The management is different and specific for both.
I suggest you to consult a dermatologist or get a second opinion from pediatrician.
Continue medication as advised.

Brief Answer:
Unlikely to be due to the vaccine

Detailed Answer:
Hi
This skin reaction is unlikely to be due to Japanese encephalitis vaccine. In reaction due to vaccine the rashes usually occur at the site or in the whole body.
