What Causes Vaginal Spotting During Pregnancy?

Question: Spotting 34 weeks 4 days pregnant. At what point should i be concerned. Pictures attached from few bathroom trips this morning.

I am Dr Ramadevi Wani. I will be answering your concerns today.

I wish you had given me some more information. Is this your first pregnancy? Do you have any medical problems like hypertension?
How did this spotting start? Was this after intercourse?
When did you have the last ultrasound? What did your doctor tell you about the ultrasound report? Where is the placenta located in the uterus?
Do you have pain abdomen or tightnings? Do you have heaviness in the pelvis? Did you start any discharge along with this spotting?

I saw the pictures. The spotting is minimal. There is no other discharge.
The reasons for spotting at this gestational age are
1.     Placenta being located in the lower uterus—This can be found out by ultrasound. This condition is serious.
2.     Placental abruption-Placenta getting partially separated from uterus . This condition is serious
3.     Due to onset of premature labor—In this case you will have pain/tightnings/heaviness in the abdomen. This condition is of concern.
4.     Cervical erosion/infection (Lesion/infection on the neck of the womb) - In this case bleeding follows intercourse. Sometimes it starts without any provocation.
MOST OF THE TIMES THE CAUSE IS SOMETHING MINOR like cervical erosion. But it is important to make sure that the bleeding is not due to serious cause.
So I advise you to consult your doctor for evaluation. She will perform general and internal examination. She will also perform ultrasound to make sure that you and your baby are fine.

Second pregnancy... Gestational diabettes, last ultrasound 28 weeks was normal. Heaviness in pubic area last couple days.... Mild crampy feeling last couple days. A couple weeks ago pink spotting with no known cause when nurse examined... Now more red drops than just pink ... Started turning more red this morning and has continued. I have not had intercourse or exam. Been off my feet last few days but still continuing so far

It is good to know that ultrasound done at 28 weeks of pregnancy was normal. So the possibility of placenta previa causing this spotting is ruled out.

Heavyness and crampy feeling are suggestive that you may probably start premature labor. You need to have an internal examination done to find out about this.

It is important to find out the cause for spotting. So please consult your doctor.

Is the bleeding still persistent? How are the baby movements?
The possible causes are as below.

1. If the heavyness and cramping is intermittent and increasing in severity it suggests the possibility of beginning of premature uterine contractions ( threatened preterm labor).

2. Do you have persistent abdominal pain? Is the bleeding increasing? If yes it raises the suspicion of small placental separation.

3. If the bleeding has reduced now, and heavyness and cramping are not increased then it could be just due to cervical erosion. You need not worry.
