What Causes Upper Right Abdominal Pain After A Meal?

Question:
Friday 9/23/16: About an hour or so after eating breakfast, I had pain in upper abdominal area on right side, just above the waist, like a fist turning inside. The pain became more intense with slight nausea and dampness, not a full sweat. Also I had really bad gas. It was so painful that I considered going to the ER or Urgent Care. Seemed to get better after bowel movement.
Saturday: about one hour after breakfast, I felt two sharp pains in right side of lower back, just below the waist and gnawing pain in right abdominal area just above the waist. A couple hours later, I had a bowel movement. Pain decreased to just some discomfort under right breast and passing of very smelly gas.
Sunday: after a late breakfast, gnawing pain in right abdominal area just above the waist, gas; then a couple of large and very loose bowel movements. Later, slight pain still happening; seemed to have moved toward my back.
Monday: No pain until after I ate breakfast. Even then, it was mostly a gnawing pain just under my right breast.
Tuesday: after breakfast a gnawing pain between right breast and waist. Saw Dr. and a 3rd year medical student. Examination. Conclusion: probably result of rectocele; need to discuss with Gyne to verify. Doesn’t think it’s gall bladder because pain is better; doesn’t want to do xray and expose to extra radiation. I was to watch closely over the next few days and if progression, fever, nausea or vomiting, or no improvement, call Dr.
Since then, the pattern seems to be as follows: Before breakfast, slight discomfort in right abdominal area, just above the waist. After breakfast, pain moves to just under my right breast (along the bra line) and is more intense. An hour or two later, I have one or more bowel movements, sometimes loose. Then the pain gets better. I have some kind of discomfort going on most of the time.
Last night (12/14/16), I had another attack. What should I do now? What should I ask my doctors after these symptoms.

I can understand your concern for diagnosed symptom.
I would like to ask you few questions first before I guide you.
1) Does pain start on front part of abdomen, on side or on back? Does breakfast aggravate or relieve pain? Does bowel movement has any association with pain? Did you have your urine to be checked? Do you have feeling of nausea just before or during pain episode?

Based on this questions I would be more able to provide more detailed information. But as a differential diagnosis I am suspecting gall stone, right side kidney stone. As i get information on above questioned, I can explain more precise.

I’ve been plagued with discomfort in lower right abdominal area for some time. Months ago the pain was in upper abdominal area on right side, just above the waist, like a fist turning inside. Lately, the pain starts on the lower right side of the abdominal area, just below the waist. Meals aggravate pain most times, but also may cause me to have a bowel movement. Pain continues the same during bowel movements, then gets better after bowel movement. My urine is checked every three months when I see my lupus specialist. The nausea is not always there during pain; it seems to kind of roll up on me during some pain episodes.

hello,
1) The characteristic of your pain does not favor the diagnosis of gall stones as pain in gall stone will be in right upper abdomen (just below the rib cage).
2) Correlation of pain with meals and bowel movement, pain at waist level suggest possibility of intestinal condition for development of pain.
3) As per your history and symptoms you should do total leucocyte count, stool examination, liver function test, USG abdomen.
4) I should put my first diagnosis chronic appendicitis or diverticulitis and second diagnosis a gall stone (based on your symptoms and history, although further evaluation is needed)
