What Causes Upper Abdominal Pain Along With Vomiting?

Question: My issue began 7 days ago. I am 55 healthy female. I munched on almonds throughout the day and had chinese chicken and rice at a mall restaurant. 1 hour following eating, I developed upper abd pain, the pain was mild and extended clear across abd in a band about 4 fingers wide. The next day I vomited the entire day, undigested food, no coffee ground, no blood. Since then I have been anorexic, extereme abd tenderness, freq belching, passing flatus, small stools daily. Still having occassional nausea, but just dry heaves. The primary pain abd pain is centered a hands breath wide starting a4 fingers below sternum and extends to umbilicus. I occassionly get severe pain 20/10 at the same area across my upper abd that spasms about every 5 minutes. I was seen in ER, no imaging, labs all normal -liver enzymes, cmp, cbc, pancreatic panal - just high ketones in urine (i had been on liquids for 6 days)....all I was told it that it is not a surgical abd...offered me dilauded for pain I declined, discharged on zofran and T#3. I started omperizole, this is horrible, I can't get into a GI because it is the Holiday week...the most i can tolerate is about 40 ml at a time....what else canI do?

Dear Ms

Going by description of your symptoms it is likely to be acid dyspepsia which is causing symptoms. As all your blood investigation are normal there is no cause for concern. High ketone in urine is likely to be Starvation ketonuria ( inadequate intake of carbohydrates causes lipids to be mobilised and utilised as energy source causing ketonuria ).

I would suggest you the following

1. Gastric motility increasing and antiemtic like Domperidone which is available as over the counter medication will be helpful

2. Continue taking Omeprozole

3. Take soft diet

4. Yoghurt or curd is a good natural Probiotic

5. Liquid antacid containing Simethicone (Anti flatulent agent ) will provide much needed relief
