Question: Hi Doctor, I am having an unusual symptoms which is yet to get an answer. Why my tingling can be passed to people? Sometimes it could happen when I talk or not talk, it's seems like came from my breath. As it's getting obvious and can happen anytime. Their feeling is just like something 'fly' to their face or around the nose to cause the itching. Sometimes they would rub their nose, or may sneezing as well. I had gone for 3 ENT but they said I've allergic.Previously also being diagnosed to have sinus infections. But my CT scan just said that I had hypertrophied turbinate and deviated septum. Doctor said my tissue is little swelling due to chronic rhinitis. But they all think my problem is not severe. They seems not believe my statement. But it's really can cause such discomfort around the people that I meet everyday. I am so frustrated as it seems can't be cured even though I took medicines and rinsing my nose by using Neilmed. I'm afraid after nasal surgery my problem still exist. Basically, I don't have serious nose blocking now in a day like previous time. Just the itching sometimes. Or nose pain in the back.REcently, my left face muscles also feeling a little tight or uncomfortable. I doubt that it's only allergy to cause me feel so. I started to think of other causes. COuld it be my nerve issue? SOmetimes the tickling is inside the nose, sometimes it seems like something 'lay on' your face but it's invisible. Thank you. Appreciate your professional advice.

Brief Answer: As below: Detailed Answer: Hi, Thank you for your query. 1. In the attached CT images, your sinuses are clear. There is a DNS with a large septal spur touching the right middle turbinate and hypertrophied inferior nasal turbinates. 2. The tingling that you experience is likely to be a Trigeminal Nerve stimulation due to irritation of the delicate nasal mucosa (lining) by the septal spur. 3. Irritation may also occur by a a build up of waste products of inflammation, infection, immune system by-products, sulphur and gases like nitric oxide and ammonia. These chemicals may also explain how your symptoms are passed on to others in your vicinity. This is however very rare. 4. A trial with amitriptyline (Tryptomer) may be done. A Septoplasty with a spur removal may help relieve your symptoms. I hope that I have answered your queries. If you have any further questions, I will be available to answer them. Regards.

Thanks for your advice. Refer to your reply no. 1 & 4: 1 , just wish to confirm, do you mean my left middle turbinate on 1st question? The doctor that I visited not telling me about this. The CT scan result that I attached is to be readed from right to left, I was told my left turbinate is hypetrophied with deviated septum only. 4. Your mean such symptoms could be due to anxiety? But I was concern about it's side effect. Besides, would Coblation better or less efficient than Septoplasty? Thank you.

Brief Answer: As below: Detailed Answer: Hi, Thank you for writing back. 1. You have hypertrophy of both the inferior turbinates, left slightly more than the right. This is a common finding. Sometimes I get a post decongestion scan done. 2. Stress causes (vasomotor) intrinsic rhinitis leading to chronic nasal congestion. 3. In your case, it is likely to be the neuralgia caused by the septal spur touching the right middle turbinate. 4. The Trymptomer (amitriptyline) is suggested for the neuralgia. It also has an anti-anxiety action. 5. Radio-frequency ablation and coblation are alternatives. However it your case, the large spur seems to be the problem. I hope that I have answered your query. If you have any further questions, I will be available to answer them. Regards.
