What Causes Tenderness And Swelling In Upper Right Quadrant With Nausea?

Question: I am a 51 y/o female. When I was in my early 30's I had elevated liver enzymes which was caused by a new birth control pill I was on for one month. I went through a series of repeat blood work back then until the enzymes went into the normal range. Last year I had blood work done and my liver enzymes were elevated once again. The doctor did a hepatitis panel on me and it was negative. At that time the Dr. wanted me to get a ct scan but I did not have insurance and I knew I would have to make payments on the blood work. So, he told me he wanted me to have more blood work, which I did and the enzymes were getting lower.
Now, I'm trying to decide if I should go to the ER. I'm not in severe pain, but I feel tenderness and swelling in my upper right quadrant. I have been having nausea at night, but no vomiting. I eat tums and that seems to always help. I suffer from feeling bloated though, even when I haven't eaten anything.
I am a non-drinker and I don't smoke, but I am over weight. I also have a history of many, many UTI's, and I have the symptoms of one now.
Your advice and/or opinion will be appreciated. Thank you.

I am Dr Anshul Varshney. I understand your concern.

The symptoms you are describing goes in favor of Cholelithiasis. There might be a stone in Common bile duct causing obstructive jaundice.

I would like to know following details:

1. Tell me your complete report of Liver function test. I want to know all the values.

2. Do you consume any amount of alcohol?

3. Are you taking any pain killers?

4. Any itching?

5. Any yellowish discoloration of eyes or urine!

6. What are the colour of your stools?

Revert with these details preferably in point wise manner.

I don't remember the values, but they were 4 times the normal limits.
I don't consume alcohol
I only take Aspirin for pain or fever
The palms of my hands itch terribly! More the left hand than the right and more severely in the mornings. ( I am right handed)
No yellowish discoloration of eyes or urine
My stools are brown

Your description says that you have Obstructive Jaundice.
I would advice you to get following:
1. Liver Function Test
2. Ultrasound Abdomen
3. Hemogram.

Get these basic investigations first. Further investigations would be planned accordingly.
