Brief Answer:
You need evaluation.

Detailed Answer:
Hi,

Thanks for your question.
Noted your concern.

With 20 years of diabetes there might be following causes for ankle swelling -
1) Diabetic peripheral vascular disease.
2) Kidney problem.
3) Heart disease.
4) Liver disease.
5) Effect of some medicines.
6) Low hemoglobin.

In such a case I may suggest detailed clinical examination including peripheral pulses & following investigation -
- Complete Blood Count.
- Renal Function Test.
- Liver Function Test.
- Urine examination including Urine proteins.
- ECG & 2DEcho of heart.
- Fundoscopy to know changes of retinopathy.
- ultra TSH.

Depending upon detailed clinical examination and above test results appropriate decision can be taken.

Hope this helps you.
