What Causes Sudden Giddiness, Shivering, Sweating And Fluctuating BP Levels?

Question: Dear Sir,
All of sudden, when i woke up my wife around 1 o'clock in the night for giving me medicine i found that she could not stand own of her own and collapsed 2 times with complained about giddiness/shivering with sweating as next step i observed her lower BP was 61 and 64 both the occasions with the gap of 15 mins internal. She drink at least 3 glasses of water and asked me to switch on the ceiling fan as she was quite uncomfortable but after 30mins again when i checked up the BP it was 76/119 this time and she was normal around 2 o'clock. It gives me worry as she never had this kind of problem in the past but her BP was always <65 range for quite few years, she is not taking any medicine for that.

I would like to know the reason behind the whole scenario which happened last night with complete line of treatment and medical tests required so that i can relate it to some problem or deep sleep syndrome due to which she could not make sense what happened. Please help and provide details urgently as i am restless.

Read and understood the whole history.
The most probable reasons can be:
Postural hypotension. This occurs due to body's inability to adjust the blood pressure required as one gets up from sleeping position to sitting/standing position. This is momentary and the sweating, and shivering are the reflex symptoms that accompany. The body is totally relaxed in sleep with far reduced blood pressure (this is normal for all).
Since there are no symptoms related to ENT, there may not be any cause related to ENT (Ear, nose and Throat).
Stress and anxiety: I was wondering what may be your problem that she had to get up at 1 am to give you medicines. What is it that you husband) are suffering from? She may be a lot stressed if you are too sick.
This may not be a deep sleep syndrome

I would advise you the following:
Ask her to consult your Physician, preferably a Cardiologist to have a General Check-up and Cardiac evaluation both.
If this was her first attack, it is better that the complete evaluation be done to find out any probable medical cause.

As per the examination done by physician the symptom is vasovegal attack and there is no cardiological test needed,pl suggest with the correct line of treatment. Do I need to take this any serious or its due to hypotension? Current she is taking multi vitamin capsules.

What is it that caused Vaso-vagal, there is usually an instigating factor like pain or so. Let us presume so. How much was the pulse rate at the time of attack?

How is she at the moment?
How much is she stressed?

It is always better to get evaluation and investigations done.
If the present Doctor is not willing, you may please second opinion of another Doctor. This should taken seriously unless proved otherwise. If the things are normal the stress and anxiety is gone and makes you both relaxed.
The specific treatment can be given only after proper evaluation.

The pulse rate was 72 [i think], she is around 40yrs now, she is stressed bit as taking iron tonic and ostocalcium twice.

I will upload the blood test report day after tommorow for getting the better clarification. Therefore please keep this open for 3-4 days.

As per consultation with Sr. Physician, vaso-vagal attack is common when you directly standup from sleep as the body temperature/BP is adjusted while sleeping becomes low and when you start walking from deep sleep that to with cold exposure then hypo-stress/body weight takes some time to adjust the BP with the temp. outside as the attack was for not more than a minute but when she started rest for sometime she became nornal.

Need your suggestions/action plan.

Well, wither of vaso-vagal or postural hypotension, she should avoid sudden getting up from bed or chair.
Anemia can be a contributory factor as is stress.
Both should be well under control.

Rest of the findings also noted.

Not to rise suddenly from sitting or lying down position.

Good 8 hours sleep is a necessity.
Continue multivitamin capsules and ad Iron capsule as hemoglobin is just above the lower limit.

I did expect this question, hence I have written above- all the important reports are normal.
All the blood cell counts including the platelet count are normal.
This is important.
PWD, MPV amd PLCR are actually calculated figures as per the analyzers in relation to the available measurement.
These are supposed to be related to the coronary (heart attacks) and the corroboration is being thought about/ studied about. Still under evaluation. Your hematologist/Pathologist alone can give you the best guidance.
