What Causes Stomach Cramps, Prolonged Fever And Frequent Defecation?

Question: Hello doctor,

My son of 8 years has been suffering from stomach problem since april 2014. It started on a trip with stomach cramps and the feeling of stool. He was put on antibiotics later followed by probiotics. After that episode his stomach never really stabiliized. I july he was on Oflox and cyclopam for cramps with mild fever. then again he felt sick this time doctor prescribed metrogyl and oflox with enetrolife for even days. He was find but again he developed frequent stools. After consulting another doctor he was put on Nizonide a week back but today again he has passed stools at least 10 times with cramps. sometimes he goes to the loo without any stool. Doctor has prescribed allergy test. He is also asthamatic and is on sereflo 50 and singular for allergy. There has not been any weight loss or vomiting. Stool test ianalysis in July indicated pus cells 6-8 hpf with some bacteria present and no RBC or blood. Please advise as to what could be the problem. He has never had such stomach problems before. Is it okay to give him cyclopam with nizonide?

Hi... I have read through your history in detail. I have a few questions for you -

1. Has he got any prolonged fever in relation to the diarrheal or dysentery episodes?

2. Is there any rash over the body or legs or hands?

3. Are there any oral ulcers?

4. Even though you have mentioned there is no weight loss - I would still like to know his date of birth, current weight and height and also birth weight please?

5. Are there any known specific foods that you have observed these symptoms to be appearing?

6. Has he been worked up for immunodeficiency - like serum Ig profile etc?

7. Is there any family history of inflammatory bowel disease or irritable bowel syndrome?

Suggestoins -

1. Stool test with pus cells 6-8 hpf with some bacteria present and no RBC or blood - is a normal report - do not worry.
2. You better avoid Cyclopam as it will temporarily stall the bowel movement and increase the risk of bacterial growth and infection locally.

Thanks for answering my query! The answers to your questions are as follows:

1. Has he got any prolonged fever in relation to the diarrheal or dysentery episodes?--No

2. Is there any rash over the body or legs or hands? -No but he has eczema which reduced considerably before this episode of stomach trouble

3. Are there any oral ulcers? - Yes only one but only occurred yesterday but healed by morning

4. Even though you have mentioned there is no weight loss - I would still like to know his date of birth, current weight and height and also birth weight please?- DOB is 5/7/2007. BW: 2.7 kg. current WT: 20 kgs, H:128 cm.

5. Are there any known specific foods that you have observed these symptoms to be appearing?-Not really but stopped milk and milk products on doctors advice for two weeks

6. Has he been worked up for immunodeficiency - like serum Ig profile etc?- He has been suggested to go for wheat allergy. Previous allergy test revealed allergy to dust mites, cockroaches, food- shrimps, prawns

7. Is there any family history of inflammatory bowel disease or irritable bowel syndrome? -No.

One important observation he has persistent nasal and eye allergy but this time noticed a greyish halo around the pupils and brown spots in the inner lower eye lids. His eyes are itchy quite often. His pulmonologist has not suggeted immuno therapy and is treating him for allergic rhinosinusitis with sereflo 50, singular and nasonex nasal spray. His bowels have become less frequent but not settled with cramps now and then.

HI...Thank you for answering all my questions - Though there might not be any weight loss, as I have expected the weight is less than expected and is falling less than 5th percentile. But his height is normal according to the age. This means that he has an acute problem which is reflecting the history which you have given that from April 2014 only he is having this tummy problem.

I put forth 2 possibilities -

1. Inflammatory bowel disease.
2. Irritable bowel syndrome.
3. Celiac disease

If I were your pediatrician I would go ahead with - the following blood tests and further refer you to consult a pediatric gastroenterologist.

Labs suggested - CBC/ ESR/ CRP/ LFT/ RFT/ Stool for occult blood and Antibody testing, especially immunoglobulin A anti-tissue transglutaminase antibody (IgA TTG)

I also urge you to consult a pediatric gastroenterologist and go ahead with a endoscopy and colonoscopy after his suggestion.
