Brief Answer:
Consult your treating surgeon to rule out the following causes.

Detailed Answer:
Dr. Hanif warmly Welcomes you!

Thanks for consulting at my virtual clinic. I have carefully worked through your case, and can realize your health worries. Being your physician, I will take care all of your medical concerns.

It happens in some patients post surgery. There are many reasons for this problem.

*Pelvic Inflammatory disease... Inflammation in pelvic area of the surgery due to postsurgical bacterial infection.
*Stricture formation due to manipulation.
*Oozing of some fluid inside the peritoneal pouches and leading to inflammation and pain on straining, coughing, laughing.
*Constipation is also one of the causes for pain of this type and after surgery sometimes constipation is a risk.
*Ectopic pregnancy (less likely in your case).

Why are they monitoring CA 19-9? Although CA 19-9 is not so sensitive, specific but it is mostly monitored to see the prognosis of treatment for chronic Pancreatitis or Pancreatic Cancer with pancreatic duct dilatation.

Your levels for CA19-9 are high definitely, normal being less than 37 U/ml.

Advice:- Consult your treating Surgeon to rule out the above mentioned causes. If I was your treating physician and PID was the diagnosis I must have prescribed you with Azithromycin class of antibiotic if not allergic to. If constipation, Lactulose syrup is good. An ultrasound abdomen is a must for you to help diagnose the cause of your illness.

Have a wonderful time ahead. Hope I answered all of your questions. Let me know if you have any further clinical concerns. I will be happy to get back to you shortly. If satisfied here, please give a five star rating and don't forget to close the discussion.
