What Causes Soreness In Inner Part Of The Buttocks Close To The Anus?

Question: Hello,
For the past three days I've had a sore on the inner part of my left buttocks cheek closer to the anus. At first I thought it was a pimple, but then I then it came from cycling too much and sitting down. I'm a little nervous that it may be herpes. I'd like some help...

- What is the intensity of pain or soreness- mild/moderate/severe.

Thank you, I attached a photo already.
It is middy painful I suppose almost like a scab. It feels like a scab as well. I have not felt any blisters to start with, but I could be wrong. No ulcers at the present. I don't know what this could be.

Friction ulcers; Do not try to scratch/touch

Hello young man,

Welcome again to the forum and thanks sending important informations as well as good resolution pictures.

After analyzing the history and correlating with your pictures I can make out two possibilities:

1.     Friction ulcers: formed due to rubbing and friction at the skin forming the superficial ulcers.

2.     Herpetic ulcers: Due to herpes virus infection.

But in my view I will keep first possibility of friction ulcers rather than herpes. It is on the basis of following important points:

1.     You can see in the picture that there are superficial ulcers. These ulcers are basically excoriations which are linear in pattern places parallel to each other. This pattern is typically seen if the area is scratched or if the area is rubbed. There is mild to moderate pain or soreness and itching is more pronounced in same.

2.     In herpes there is typical vesicles are formed and these remains for 1-2 days and then forms the ulcers. These ulcers are multiple, rounded with deep redness around it. There is moderate to severe pain or soreness and itching less in herpes.

3.     If you can notice the sore areas are present quite away form the anal opening on the buttock proper. In herpes the lesions are present near to the anal opening.

So you probably got some idea regarding your condition. Even then to confirm and rule out herpes I generally suggest going for IgM and IgG antibody test for HSV1 &2.

I also request you to kindly tell me any history of unprotected intercourse especially anal intercourse in last 2 weeks or prior to that.

For the treatment part- In your kind of cases I suggest following important suggestions:

1. Avoid and hot – humid climate.

2. Wear cotton clothes and avoid synthetic clothes.

3. Stay in cool environment (preferably AC) and avoid sweating.

4. Take two time bath once in morning and at night time which will soothe your skin and prevent the aggravation of the problem

5. Do not Pinch pop or squeeze or rub any area.

6. Do not use any deodorants or any cosmetics over and near the affected area.

7. Do cold compression over the affected area. Take a wet towel soaked in cold water and apply it for 5-10 minutes for 3-4 times a day.


I suggest my patients to apply an antibiotic craeam such as Mupirocin or fucidin oer the affected area. Apply in thinnest possible amount for two times a day. If there is any irritation or itching is present then can take tablet “Loratadine” or "Deslotatadine" tablet by mouth up to two times.

The most important part is “No Touch” to the affected area and no further friction to the site for earliest healing.

You will be alright in next couple of days. My best wihes for early recovery.
