What Causes Small Red Patches On Head Of Penis?

Question: Good day
57yr old male- no previous issues-general great health.
I have a rash on the head of my penis. Under the fore skin as I am not circumcised.
Started as small red patches. 1/4 in to 1/2 inch sq.
Did full cycle of Cipro and it disappeared. Two days later it reappeared in great concentration and is now showing splotches. A few open and sore, bleeding if stretched and cleaned in one small area. Easily 50% more present.
I've been through 2 full batteries of testing. Everything they could think of. All negative.
Shingles, All STD, everything we would think give the presentation of symptoms.
Doctor treating me in TX suggests seeing CD Specalitists. 2 to 6 weeks in XXXXXXX market.
Went to UC ER in XXXXXXX OH yesterday at his suggestion.
Doctor says yeast infection. Proscribed Prescription version of Lotrimin cream.
This has left me even more sore and seems to be of little effect first 36 hours.
Seems like my first doc would have ruled out yeast early on. Skin and urine cultures taken again all negative.
Please help.

I have gone through your medical history carefully and understood your concerns.

1. A picture of the rash in the head of your penis is needed for a more specific opinion.

2. You have been taking oral cipro and the rash improved. This is indicating that the rash is due to bacterial infection and not yeast infection.

However, a good photo will help me give a more precise opinion.

3. It is common for uncircumcised men to have rashes or infection of penile head (we call it balanitis).
To my opinion, you need:
- another course of oral antibiotics
- keep a good hygiene of your genitalia
- avoid using scented soaps
- apply topical antibiotic cream (containing gentamycin and hydrocortisone) to relieve the pain

After this acute episode will be gone, you should consider circumcision as it is the final solution to your problems.

I also advise to get further tests like measuring fasting blood sugar and HbA1C to rule out diabetes.

I advise to continue following the advice as given earlier. If no improvement with local treatment, should consult with urologist to start oral antibiotics.

I highly recommend to seriously considering circumcision as it is the definite solution to your problem.
