Question: Need your help
I am a 50 year old man with many health issues, so I am a complicated patient.
Obese XXXXXXX with moderately elevated liver enzymes
Type 2 Diabetes
Severe Sleep apnea with CPAP
Hypertension finally controlled
Stage 3 CKD
Lipid Disorder, Very high Triglycerides and low HDL
Anxiety/Depression
Right sided HF with normal systolic EF
Drugs

Here are my drugs:
HBP- Irbesarten, Carvedilol, Epleronone, Clonidine, Furosimide
Lipids: Crestor, Lovaza, Tricor, but Discontinued recently for elevated liver enzymes.
Diabetes: only insulin due to my kidney issues. Lantus bid and Pen fast acting insulin before meals.
Zoloft, Hydroxizine ASA 325,
Xanax PRN
My Diabetes is not controlled well and I am headed towards weight loss surgery.
I am also on Neurontin for carpal tunnel and for pain in my toes.
I have had a Nerve test and was told no diabetic Neuropathy, but carpal tunnel in both hands. So my hands are usually tingly or numb. I have general fatigue. My muscles ache, especially my biceps and my hand joints ache and recently. I have had a million tests and no RA factor. My right toe is killing me, but told it does not seem or look like gout. All of my toes have a deep ache. My vision is more blury lately, but figured it is my Diabetes uncontrolled as well as my tiredness. I have also complained of poor memory & slight confusion at times over the last year. Told I am stressed and on alot of meds. Maybe I am just paranoid, but i have not been tested for HIV since I was married. Have had some unprotected sex. Does this sound like HIV or anything else u can think of?
Thank you, XXXXXXX

Brief Answer:
Testing for HIV only can rule out or confirm HIV. symptoms are not typical

Detailed Answer:
HI,
Welcome to HCM.
Thanks for posting your query.
You are having multiple problems including Diabetes, Hypertension, CKD, Triglyceremia, elevated liver enzymes, borderline D vitamin deficiency and Depression.
Your symptoms are not specific for HIV. It can be due to various other reasons. You have to see serum uric acid level for your joint problem.
Regarding your unprotected sex, test for HIV only can either confirm or rule out HIV. The symptoms described by you are unusaul in HIV disease when compared to fever, cough, weight loss, lung infection, diarrhea etc.
So have the HIV test and rule out HIV.

Thank you for your answer on HIV test. As far as my myalgias, I have always on and off had sensitive muscles. My joint and toe pain shiukd be helped by my Gabapentin. Between my achy musles, big toe pain, overall burning pain in my toes, numbness in my hands, etc, it feels like my body is being attacked.

Brief Answer:
Myalgia and other neuritic problems may be due to Diabetes

Detailed Answer:
Hi,
Welcome back.
Diabetes itself can cause all problems or aggravate the existing other problems. Unlikely to be due HIV. Test for HIV only can rule out or fix HIV. Injection BI,B6 and Methycobalamine combination for a period of 2-3 months will do better for you.

Brief Answer:
Useful in diabetic neuropathy

Detailed Answer:
Hi, Welcome back.
Any family physician or ER doctor or an internist (general physician) can give the injection. In Diabetes nerves and muscles would be affected due to poor blood supply. For this condition, the combination of vitamin injection will do good.

Could any of this be latent syphillis?
doctor
Answered by Dr. Sankaranantham Murugan (4 hours later)
Brief Answer:
Very much unlikely to have these symptoms in a latent syphilis.

Detailed Answer:
Hi,
Welcome back.
Syphilis is a great imitator. It can manifest like this as it can practically involve any system of the body. But this is all old stories. We are not seeing any florid manifestations of syphilis for the past two decades. Especially in latent syphilis. Moreover when these symptoms are due to Syphilis we can not call it as latent syphilis.
Only way to rule out Syphilis and HIV is only through appropriate blood test for these diseases instead of guessing the possibilities.
