Question: My belly button is beginning to stick out and I sometimes get a sharp pain in my right testicle. Could it be a umbilical hernia?

Yes, it may be a sign of hernia indeed. Especially if it is more visible when standing and less visible when lying down. Pain in the belly button can be present from time to time.

But the pain in the testicle is not characteristic for umbilical hernia. More possible are varices of spermatic cord, testicle inflammation or even inguinal hernia.

I suggest you to consult general surgeon with umbilical hernia issue. Testis should be checked by urologist and ultrasound should be done.
