Question: Hello Doctor

Since 2 days i dont know what is wrong with me. I need to exert more force to perform a physical activity like walking even and i am also experiencing body shake in middle of any physical acitivity. Even when i am sitting idle my body shakes a little. my legs and arms muscles get tired very very easily. I think its asthenia because all my reports(sugar, lft kft, ecg, eco, hotler monitor, ct scan, tft, pth, lipid profile, b12, folic acid, cbc and vitamin d3) have been evaluated which were normal. Only my vitamin d3 was low and i am taking d3 capsules for it. Please give me advice on what is to be done. I am having a heard time dealing with my body. I want to feel better and i want to recover from this uncomfortable illness. I never experienced this kind of problem before. Please help me by sharing your views on this condition.

Brief Answer:
Low vitamin d3 also can give fatigue.

Detailed Answer:
Hi

Thank you for asking HCM

I have gone through your query. A low vitamin D3 also can give muscle fatigue. Persistent anxiety also can cause feeling of weakness. Chronic fatigue syndrome due to some inflammatory process in the body is another reason. This can be sinusitis , tonsillitis or even tooth problems. Complete blood count will show signs of inflammation like raised ESR , leucocytosis etc in such case.
Please upload all your lab test reports if possible.

Meditation or breathing exercises will be helpful. You an also do resistance training or cardio work out. You can continue taking vitamin D3 supplements. Along with it taking vitamin B12 and levocarnitine is also good. Vitamin b12 deficiency is below 200pg/ml. If it is between 200 and 500 also taking b12 supplements is good.
Avoid caffiene , coffee , smoking and alcohol also.
Get 8 hours sleep daily. Take food half an hour after work out and not to delay more than 45 minutes to take food.

Let me know if anything not clear.
I am happy to help you.
Thanks.

Hello Doctor

Thank you for the reply. Earlier also i had low vitamin d3 but never i experienced this kind of weakness. One more thing i would like to share is i had an ABG test 10 days back. My co2 levels were low due to which the blood calcium was also low. I suffered respiratory alkolisis and my doctor advised me to breathe into paper bag to restore the co2 levels. Further he WARNED me not to take any form of calcium supplements to increase calcium levels because once co2 is restored the blood calcium levels will balance on its own. I was fine for a week but now since 2 days i am feeling severe weakness.

You have advised me to do resistance training which is not possible in this situation because my body cant perform high intensity workout at this moment. I am stuck in dilemma and i dont know how to overcome this weakness. I cant take any calcium supplement also because it will do more harm then good. taking supplement will cause hypercalcemia...My doctor says keep breathing into paper bag and u will be fine. What are your views. Please let me know

Brief Answer:
Hyperventilation due to stress can be the most likely reason.

Detailed Answer:
Welcome again.

Thank you for your follow up query.

I have gone through your reports. All are within normal limits except vitamin deficiency as you said. It is better to do ESR (erythrocyte sedimentation rate) and C reactive protein in blood to rule out any rheumatological problems also.

Hyperventilation syndrome caused by stress or anxiety could be your problem most likely. Recycling the breathed air will be helpful for this. Breathing on paper bag is good for this. If this is not helping then congnitive behavioral therapy by psychologist and a short course of benzodiazepines will be helpful. This will help to normalize your situation.

Yes, Anxiety can cause such symptoms. Anxiolytic dugs along with behavioral therapy by psychologist will be helpful for this. A consultation with psychiatrist and psychologist is well appreciated for this.

HI DOCTOR

i have heard of diseases like fibromyalgia and muscle dystrophy . I hope i am not diagnosed with any of these disease. Can we rule this out in my case because i not experience pain..i can run even but sometimes when i am idle my body experiences a mild tremor. It happens only when i am idle. I think i m experiencing perceived weakness, something that dont exist but my mind has perceived it. This has happened because i have overly stressed my body.. Whats your view on FIBROMALYGIA and MUSCLE DYSTROPHY and PARKINSONS DISEASE.

Brief Answer:
Explained below.

Detailed Answer:
Welcome again.

Thank you for your follow up query.

You are too young to get parkinson's disease. Even the rare young onset parkinsons disease can start at age 40 only. Fibromyalgia is a condition of having painful (tender) points on various parts of body with out a reason. That is also most unlikely in your case. When going through all laboratory values you are not likely to get muscle dystrophy also. Physically you are having all healthy parameters. So it is most likely anxiety and stress which causes these symptoms.

Behavioral therapy and various exercises by psychologist is all you needed in this case. Anxiolytic drugs also helps along with this. So no need to worry about other problems. You will be all right soon.
