Posted on Fri, 12 Sep 2014
Question: My 11 son continues to have severe pain in and around naval for last 6 mths. All reports of endoscopy, colonoscopy, Barium Milk X-Ray, CT Enteroclysis, Ultrasound, Video Endoscopy, X-Rays are negative, all blood tests for C3 & C4, Lead etc are negative as well. He was diagnosed of ITP about 9 mths back but that always remained a question (?).
He had Dengue in Dec-2012 and was wrongly treated of Dengue + Typhoid once again in June-2014.
We are based out of New XXXXXXX and tests were conducted at Fortis Hospital, XXXXXXX & Asian Inst of Gastroenterology @ XXXXXXX
Please help.... thanks

Brief Answer:
Mesentric adenopathy possibly appendicitis induced

Detailed Answer:
Hi
Thanks for writing to us.
I have seen all your reports.
The pain is probably due to mesentric lymph node swelling
Your child has been fully evaluated and treated well so far.
Appendicitis can cause mesentric adenopathy ( which biopsy report proves )
After removal, the mesentric nodes will go down in size slowly.
The pain will also subside but may take one or two more months

Continue regular followups as advised

Request you to run thru the first few sheets of "Aryan AIG_2 Reports.pdf" which has been uploaded later - there is no lymph node or fluid and the intense pain still continues till date.
What are the chances of (a) Vasculities (b) abdominal TB (c) intussusception and how can they be ruled out?
He has been having ULTACET continuously since last 5-6 mths; how safe is it? Can you prescribe something safer?
Is there any doctor or hospital within the country or outside who may be specialized in treating such a case?
More than a GASTRO issue, does it look like a HEMATOLOGY issue?

Brief Answer:
Chances are less

Detailed Answer:

I have seen that report : clearly mentions lymph node - autolysed few showed reactive changes

Vasculitis is less likely as the workup for that ( ANA , biopsy, etc ) has been negative
abdominal TB was negative in laproscopy biopsy and quantiferon
Intussussception cannot be missed in ultrasound and CT
Ultracet is safe , dont worry
I still feel it is gastro issue,
You have been to two best centers in XXXXXXX
Cant recommend anything better than those as far as i know
Regards
