Most important and frequent cause of rectal bleed in adults accompanied with pain are Anal fissures or Hemorrhoids or Severe constipation.

Pain in right iliac fossa is often encountered in patients suffering from constipation.Please mention your bowel habits. Constipated patient strains a lot while defecating leading to rupture of rectal mucosa causing mild to moderate bleeding.

However if you are not suffering from constipation then a physical examination should be done by your primary to rule out the possibility of hemorrhoids(Proctoscopic and digital examination would be done) and anal fissures.

Please specify your bowel habits.Further suggestions could be given only after that.
