What Causes Severe Nasal Congestion At Night While Suffering From Cold?

Question: I've had a cold for 2 1/2weeks. I am definitely better but still coughing up phlegm 5-6 times a day. I become congested at night for the last 3 nights. My concern is that my cold doesn't go away.

Well it is certainly miserable to have a cold for this long.

If you aren't having any fever or chills, and the secretions are mostly clear (except after you've been sleeping for awhile), then it is most likely still a virus, and will go away in time.

By congested at night, I will assume you mean nasal congestion. If this is not correct and you mean chest congestion, please let me know.

Now let's look at what might help and maximize the effect you get from the meds you take:

If you are not having fevers, chills, or pain, I recommend skipping the XXXXXXX Seltzer formulation as it has some things in it you don't need (for fever and pain) and probably not enough of the things you do need (mucus thinner, cough suppressant).

Here is what I recommend at this point:

Flonase or other steroid nasal spray. This will take a few days to kick in but will help with swollen nasal passages, and may help with drainage that may be causing the productive cough.

Salt water irrigation. This can be via nettipot, or ready made saline spray that you can purchase, such as Simply Saline. Don't use it within a half hour or more of the Flonase, or the Flonase will end up dripping down into your stomach. Consider washing your sinuses before bed.

If the mucus is thick and hard to get up, guaifenesin can be helpful. It is a safe old drug and works if you also drink a lot of water with it (2 glasses). It draws the water to secretions, thereby thinning them and allowing them to be moved out easily. Mucinex is one brand for this but it is also sold as generic.

If the cough is keeping you up at night or interfering with things during the day, dextromethorphan is a cough suppressant.

Now about the night time nasal congestion: phenylephrine is a decongestant that you can take at night time - it isn't stimulating. But if you've used pseudoephedrine before and it hasn't kept you awake at night, try that. Pseudoephedrine does a better job of decongesting, but it can be stimulating so be careful with taking it at night. To get pseudoephedrine these days, you have to ask for it as although it doesn't require a prescription, it is kept behind the counter.

But I'd stop the XXXXXXX Seltzer and the ibuprofen if no fever or pain.
