What Causes Severe Lower Back Pain?

Question: Hi,


This is 3rd time in 3 months that I am having the following pain description.


Last night while sleeping I woke up with severe pain in the lower back (near the hip), I realised that I was arching my back a little too much which triggered the pain. 


I don't have pain when I lie on side, but feel pain when I am lying on my back.  The pain is really bad when I try to stand from sitting positing and try to sit from standing position and pain when i try to walk in brisk pace. 


It feels uneasy when I am sitting, but not very painful. 


The first this happened 3 months ago (I can't pin point what triggered it but it was after waking up). I went to the physio who suggested that it could be disc but in 2 days time it completely resolved itself (My stomach was also bloated that time, i thought it could have because I had some gassy food).


The second time was last week again after i woke up, resolved itself in 2 days, again that time as well my stomach was bloated and i was burping the entire time.  Please see the attached image, the line is where the spine is and circle is where i have the pain (It appears that the circle is where the hip bone is). 


I have acid reflux problems and as a result, i don't take medicines unless it is necessity as it upset my stomach


I am 36 old male and 4 years ago i had an attack of cervical spondylitis

Pain due to spasm caused by Multiple reasons.

Detailed Answer:
Hi , I am Dr Gopal Goel Orthopedic Surgeon.
Since I cannot find any pic attached to your query I will go by your description.
This kind of back pain ,occurring at night , during change of posture , and walking at brisk pace- is due to muscular spasm. The cause of muscular spasm has to be found.
the muscular spasm could commonly be because of slipped disc ,lumbar spondylosis( like cervical spondylosis), or sacroilitis. I would suggest x ray of L S Spine and better still would be MRI to find the cause of spasm.
I would start mild pain killers , muscle relaxants and Spinal Extension exercises to build up your muscles. Since these are repeated attacks , lasting only about 2 days, I would , in my practice,give exercises for long term use and the pain persists after few months then go for MRI.
