Posted on Mon, 8 May 2017
Question: Hello doctor,

My name is XXXX and I have been suffering with dry tight lower lips for more than a year for which I use Vaseline in large amounts in my lips. Last month I got some severe dry lips like which led to fissures and peeling lips and the dermatologist in this forum as well as the personal dermatologist whom I met suggested that it could be due to some allergy or bacterial infection and gave me antibiotic and certitude for one week and told me to continue with cetrizine for another week along with hydrocortisone acetate cream to be applied on lips I did so and it was ok . Now for the past two days the vermillion border of my lower lips are red and when I look close to it I could see pimple like but only that when I touch it it's plain surface the doctor said it could be Fordyce spots but will that become red . Also I get the sensation that there is some fissures like in my border but I can't find any . For the past one month I have stopped applying Vaseline vigourosly and use it only if it's necessary . I am sooooo scared of herpes that I have taken two blood test last six months ago and it came negative . Also I have not had any lip to lip contact with anyone . I am very scared . At times this vermillion border burns but that's when after I rub it as there are always lots of oily dirt around it . U am attaching the pics of last two days and before that for your reference please help me

Brief Answer:
Use triamcinolone oral paste

Detailed Answer:
Hello
Thanks for the query and photographs.
Your lips appear to be perfectly normal.
Dryness of lips happen due to loss of water from skin covering lips.
Take plenty of water.
I treat such cases with a mild steroid cream like triamcinolone oral paste to be applied twice daily for a week and Capsule Vitamin E 400 mg daily for a month.
This combination works well.
You can try this.
Let me know if you have any other doubts.
Thank you.

Hello doctor ,
Then why do I get this redness in my vermillion border also accompanied by itching or burning at times ? If you happened to notice the pics that I sent you the left side of the vermillion border I could see one pimple like but when I stretch it it's just flat surface but two days before there was some mild pain in that area now I don't know whether the pain was in that area or in my lower front teeth as I have gum problems for many years . I am sending you some more pics please do let me know what to do . I am drinking almost 3 litres of water daily . But for the last two weeks I was outside on business trip so had only hotel food which was mostly paneer and cheese could this affect my lips

Brief Answer:
Use triamcinolone

Detailed Answer:
Hello
Thanks for the follow up and photographs.
I can see a bit redness in central part of lower lips.
It is due to allergy.
Triamcinolone should be of great help to you.
Continue taking good amount of water.
Even in hotel you can opt for salad and curd based preparations rather than paneer.
Let me know if you have any other doubts.
Thank you.

Hello doctor,

So do you think that the redness and irritation could be because of my food intake . Cause I do eat a lot of paneer throughout until now . I am sending you the pics I took few minutes ago . Now today there is much of dryness and flakiness throughout my lower lip. I have sebamed lip balm with me but I am scared to use it . As of now I only have Vaseline and hydrocortisone acetate cream with me only tomorrow I can go out and buy the other one recommended by you . Please let me know what can I do and also this redness I always get it immediately after I have food any thing even if it's home cooked balanced food then the redness goes away after sometime .
