Question: Dear Doctor,

My wife(54yrs) with 3 children aged 21,16 and 14 yrs had lost her mother in XXXXXXX last. after two months she started talking about rebirth and reincarnation and Hindu religious books when a Neuro Psychiatrist was consulted. She was admitted ito a hospital under his supervision. Drugs administered ;
1. Inj. NEEDOL (1amp) + Inj PHARGAN (1amp) 1 amp Stat & BD
2. Tab OLIMELT 10 1TAB MS
3. 10% frutodex in saline c MVI, 1 BOTTLE DAILY FROM 14/08/2015
FOR THE FOLLOWING SYMPTOMS: SLEEPLESSNESS FOR 4 NIGHTS, IRRELEVANCY AND OVER
RELIGIOUSNESS with PRESENCE OF FAMILY HISTORY

She was released on 19/08/2015 with SATISFACTORY condition and was prescribed
i.Tab OLIMELT 10 1 TAB AT BED TIME, II. Tab BEXOL 2mg 1 Tab after Breakfast and
iii. Tab VIBRENIA 1 Tab Daily
She was better and with no symptom. On followup on 21/09/2015 she was prescribed the following drugs: a) Tab Flurilept 1 Tab once in a week b) Tab VIBRENIA 1 Tab Daily
After TWO weeks she was restless, weak, started detesting food and was afraid whereupon
she was prescribed the following drugs for c anxiety : c) Tab ARIP MT 1O at bedtime d. Tab CENSPRAM 10 - 1 TAB AFTER BREAKFAST AND DINNER e. Tab ZEPIZ 0.5 - 1 Tab at Bedtime and f. Tab VIBRENIA 1 Tab Daily after which TWO days later, she complained of burning sensation inside her body , dryness in the throat, difficulty in speech with restlessness and lethargy. Tab ZEPIZ was stopped and was used SOS. However, the complains of burning sensation inside her body , dryness in the throat, difficulty in speech with restlessness and lethargy continued. Next prescription was changed to the following drugs: a) Tab ARIP MT 1O at bedtime b) Tab CENSPRAM 10 - 1 TAB AFTER BREAKFAST AND DINNER c) Tab ZEPIZ 0.5 - 1 Tab at Bedtime d) Tab VIBRENIA 1 Tab Daily and f) ETILAM 0.5 1 Tab at Bedtime. from 15/10/2015. But the effects continued and intensity increased. Thereupon ETILUM and ZEPIZ was stopped from 18/10/2015 i.e. after THREE days. ZEPIZ has been used (HALF Tab) SOS.

Now patient is suffering from speech difficulty. weakness, lack of desire for food, restlessness.

Kindly advise

I think there is need to go back to Olanzapine

From the available description it appears that these symptoms appeared after discontinuation of Olimelt (Olanzapine) in addition to start of Arip MT. Arip Mt and Olimelt work by similar mechanism but have very different side effect profile. In addition effectiveness of one medication do not prove that other will be helpful.

The other reason for her symptoms could be Flurilept which is a long acting medication and remains in the system for long duration. The effect may last for one month to three month.

If she had been my patient I will go back for Olimelt while stopping Arip MT as it will be the most likely thing to yield results.


I will also like to evaluate her for ischemic infarct as they can occur simultaneously at this age. For confirmation a MRI of brain is required so appropriate treatment can be started if there is any evidence of stroke.
