Brief Answer:
Local inflammation of the skin on glans!

Detailed Answer:
Thanks for the consultation at my virtual clinic HCM!
I have carefully worked through your case, and can well realize your health worries. Being your physician, I will take care all of your medical concerns today and will put my best efforts to get you out of this situation.

Please don't worry as this seems to be a mild skin inflammation of the glans penis. You need to take care of the genital hygiene. Never go for unprotected sex as it may cause many sexually transmitted diseases. Also avoid multiple sexual partners.

Had you have sexual intercourse recently without using condom especially with some unknown female whose health status may be unknown to you??
Do you also have an itching or burning sensation on the glans??
Do you have any issues with the inner thigh or scrotum skin??

In Medications you don't need much. Just keep applying Neosporin to avoid secondary infection by the bacteria.
Take Ibuprofen to relieve this inflammation and redness.
If you have recently changed your soap, don't use that. You may take a shower with some mild unscented soap like Dove.
Wear cotton only underwears and wear loose pants.

Hope I answered all of your questions. Let me know if you have further clinical concerns or doubts. I will be more than happy to get back to you with expert opinion. Please don't forget to give Five Star rating to help me assist my patients in a better way; and, please don't forget to 'Close This Discussion'.

I am worried so much and looked on internet and came across many terms such as zoon balanitis, lichen sclerosus and even scc (cancer) and I started comparing images. There is small ulceration

I did not have unprotected sex for months. I don't know if this is fungal infection. Another reason I am worried is that I have oral lichen planus, I read some people get that on penis, however I see no white straie lines or for that matter any whitish plaque or rings, so I don't think it looks like lichen planus.

Please keep in mind I worry a lot and many pictures on internet have scared me because this is the first time I have this infection. I don't know if this is auto imunne condition or just mild irration. Is it balanitis ? what is it afterall ?.

Please clarify. I also read if you just apply neosporin (anti bacterial) and the inflammation is due to funfgal, it can get worse and more red. What should I use anti fungal or anti bacterial or combination of anti inflammatory, anti bacterial and anti fungal

How long this will take to go away ?. I am worried if it ever. The oral lichen planus is almost year now. It has not gone away yet. All these and fear of cancer is worrying me a lot. The internet has made it worse and I am unable to come in terms with what's going on and confused. My general knowledge of disease is too much and reading every diseases type has made it like I don't even know what is this ?, or is it just mild irritation ?. I compared all pictures and tried to match if it's this or that. I feel everything is possible. I also noticed the ring gets red after bathing and feels like red band across the penis.

Please advise

Brief Answer:
No need to worry much. Start the management aforementioned.

Detailed Answer:
Welcome back.

Please don't get scared as this is not looking like to be SCC or cancer.

Don't get panic from the word Balanitis. It only means infection of the penis head. Just a term used to identify the infection, like we say Pharyngitis for the infection of the Pharynx. So importantly worrisome is not the infection. It is the type and severity of the infection that should have to be the concern.

So it actually is the inflammation of the skin and may most probably be due to a bacterial infection. We don't need to put unnecessary creams on it. Just follow the instructions aforementioned and you will be better soon.

For throat infection you need to consult an Otorhinolaryngologist for thorough evaluation and treatment.

If information on the internet was of this much use, everyone was able to treat after diagnosis by himself. But this is not the case. Don't indulge much in conclusions about the information on web.

I did masturbate few times , once every 3 days for past few weeks. I wonder if it's due to that, may have created a tear and then inflammation, also humidity, sweat and lack of cleaning may have made it worse, again this my guess. The photo I sent earlier looks like small red patch, however it later changes to red band along the top side after showers. If it was small red patch , that could be due to friction and tear and then inflammation. This red band at the top along the width of the glans worries me. The skin feels different and thicker, pinkish there compared to rest of the glans. It could be due to inflammation, not sure.

Anyway, I will apply the neosporin and wait for the improvement. How long will it tale to clear. The red patch is getting bigger with superficial ulcer in that red area. I have no pain, no burning or pain in the groin due to lymph nodes.

Brief Answer:
Neosporin will help prophylactically for preventing secondary infection!

Detailed Answer:
Welcome back with the query!

No masturbation has nothing to do with the issue. Yes as aforementioned, genital hygiene is very importantly needed for you. You need to keep the area shaved timely, go for protected intercourse and avoid multiple sexual partners.

Change in appearance after showers is nothing to worry. It may all take upto week or so to get better. I am always a one click away from you for any help you need.

so you are sure this does not look like Auto immune disease like zoon balanitis, or lichen planus or lichen scelorous. It's more like bacterial infection. That will relieve all my worries. I am worried more if this is auto immune condition because they may have small risk of SCC, they are also hard to cure and tend to be chronic, create scarring etc

Please confirm

Thank you

Brief Answer:
Don't worry, it is not as you are thinking and not leading to SCC too.

Detailed Answer:
Welcome back with a follow up!

Autoimmune diseases sometimes affect like this but they also have other concomitant manifestations too. Don't worry, this doesn't look like to be due to autoimmune disease at this point of time. And I see little to no chances for the SCC too.

Start the treatment aforementioned and we will see in the days to come how it proceeds. It will also clear and rule out many of your doubts and we can proceed further towards your better health. Avoid sexual intercourse until you get better.

I have added two more pictures, pictue 1 and picture 2, please take a look. I see sharply demarcated borders and I wonder if it is zoon balanitis which tend to have similar characteristics, and it is immune disorder. The sharp borders are visible when I clean and dry it. Or it is just simple inflammation ?. However I noticed thickness of the band has reduced and I can see wrinkles back on the penis head, is it improving or getting worse ?

Should that be a concern ?

I am applying neosporin

Brief Answer:
Not Zoon Balanitis. It is improving now means effective treatment!

Detailed Answer:
Thanks for the consultation at my virtual clinic HCM!
I have carefully worked through your case, and can well realize your health worries. Being your physician, I will take care all of your medical concerns today and will put my best efforts to get you out of this situation.

Clarification:-
No it doesn't look like Zoon balanitis (balanitis circumscripta plasmacellularis OR plasma cell balanitis). There are clear differences there: The diagnosed patients at large are uncircumcised, they have yellowish hue with pinpoint purpuric manifestation, named "cayenne pepper" spotting. A lot of other symptoms that I ruled out in the very start of our discussion were actually to rule out these concerns which you are showing right now.

For example if you don't get cured and or still you have doubts, we may go for skin biopsy and some other tests like reflectance confocal microscopy (RCM) to confirm the diagnose that. We have then treatment options available if confirmed for instance as to be zoon balanitis. But right now it is unnecessary to go for that.

Some immunity boosting foods that will help you get better soon are:-
Red Bell Peppers
Broccoli
Garlic
Ginger
spinach
almonds
Yogurt
Cashew
Turmeric
Green Tea
berries
citrus fruits
kiwi
apples
red grapes XXXXXXX
onions
spinach
sweet potatoes
carrots
Old fashioned chicken soups
Shiitake Mushroom
These foods have got miraculous effects on patients of this category and I have a wide variety of experience on observing improvements in my patients. Most of the patients name them as the magic foods.

I have reviewed your pictures and Visibility of sharp borders are due to the fact that when you clean up the debris of the skin, it shows clear margins of the lesion below.

Indications of effectiveness of the management :-
*Reduced band thickness.
*Wrinkles back on the penis--Glans inflammation had made it tense and the skin wrinkles were gone. Wrinkles back is the main indication of improvement and now the process of healing will speed up.

Continue management as advised and you will feel much better.

I also read about Erythroplasia of Queyrat (EQ) which is true form carcinoma. It gets really confusing with all the internet pictures, it feels like zoon balanitis, EQ, internet makes it worse. I am glad I contacted you because by uploading picture I can show what's going on and if there is healing or not, earlier I had small ulcer in the center, but there was no band, check yesterday's picture, it's gone now. Do you think applying neosporin is making it red band ?

Since I never had balanitis before I don't have idea if this is something different type balanitis. I am comparing only band like structure, shiny and smooth surface and dark redish/pinkish appearance and demarcated border. I have uploaded correct picture picture 10 just now. Please take a look, it is not clear if it shows healing or band are forming. The band size is same and borders are much sharper than yesterday, it's just that I see now small wrinkles on that band and earlier it was thicker rough, now feels smooth and see aligned with the rest of the penis skin

I am thankful to you for your suggestions. I hope this band pattern with borders clears, that will take worry of my shoulder for good.

BTW I am uncircumcised.

Brief Answer:
Not Erythroplasia of Queyrat. You are getting much better with time!

Detailed Answer:
Welcome back with a follow up query. Happy to answer your doubts.

No this is not an Erythroplasia of Queyrat (Bowen Disease of the Glans Penis). The presentation and other symptoms are all different and not fit into that diagnosis. I suggest you to kindly stop reviewing different diseases on internet as it will confuse you more.

Banding and demarcation will merge soon with rest of the skin of penile glans. So don't bother about that. Ulcer of cancer presents differently too. That is erosive and bleeds mainly, itches and crusts badly.

Red band is due to the inflammation not due to application of neosporin. It is getting better with time. I have also reviewed pic. 10 you just uploaded. Band pattern is nothing but demarcated inflammation of the local skin.
