What Causes Rectal Pain When Having History Of Hemorrhoids?

Question: having 7-8/10 pain around rectum. have had hemrroids flare up before but usually they disappear in a couple of days. this has been about a month nowand is becoming more painful on a daily basis.

In view of the painful hemorrhoids for a month, in My opinion You seem to have
External Hemorrhoids?or a Painful Fissure in Ano?

For this to be resolved You please update more details-as follows-

Is the pain with bleeding and throbbing ?
and what is its frequency?
Is there any painful skin tag with streak of blood or splash of blood ?
Is it with malaise and fever with Constipation ?
What sort of food - are you taking ?
Any recent partying or hotel or outside street fast food ?
How are your bowel movements?
What is days job and diet schedules and life style?
Update any stool test / blood test reports done lately?

Please update this and more information, to review and study about your health issues in next session.

Do's Suggested-

Avoid Spicy and fried diet.
Be on liquid diet for 3-5 days
Take Sitz bath
Rest with lowering of head end if pains are more.

#1. some throbbing,yes but not always. some minor bleeding, more like spotting, very minimal bleeding. #2.frequency is all the time now. no skin tag,no malaise or fever, no constipation. #3. regular diet as far as I know, fruits,veggies, meat, don't eat a lot of junk foods, no mcdonalds or fast foods.no street food, no partying(imdull by colleges standards) #4. bowel movement once every twenty four hours. #5. I work in EMS,and didn't want to bother the ER doc's with my problem. shifts are a bit harsh,12's, 24's, 36's hour shifts on a weekly basis.always on edge,so to speak in regards to readiness to go at a moments notice, don't really sleep well on the job, but do get naps and make up for lost sleep when I'm off shift. no stool or blood tests yet. thank you

Read and reviewed your updates in context to your health issues of persistent painful hemorrhoids for last 1 mths time.

My Impression and Comments -
As things are with blood spotting along with pain, your infected hemorrhoids are heading to -Strangulation STAGE with thrombotic complication in time to come in near future, if things of job stress remain like this and if don't take proper care in addition to Do's suggested above.

Specific Care-Suggested-
Lifestyle Change with Diet Change with proper REST- is a MUST
Besides above preventive steps suggested in last reply,please follow-
High Fiber/ liquid bland Diet
Bowel Evacuants with Tab Dulcolax if hard stools are recurrent problem, to avoid aggravation of painful hemorrhoids.
REST with Head Low position(Trendlenburg Position) during resting period for 8 hrs /
Cap Dafflon-500 mgm x 2 per day(Venotonic) to control Engorged veins.
Antibiotics like Tab -Ciplox Oz- to control local infection
Tab Motrin-400 mg x 2 per day
Tab Tylenol-500 mg x 2 per day for pain control.
This would avoid surgical procedure.
Surgical remedy-
If need be Consult Surgeon to plan for needful Surgical Correction-By Anal Dilatation- with High ligation of hemorrhoids, under antibiotic cover, after 1-2 weeks of conservative treatment to reduce the infection before surgery.
