Question: Hi Doc...I have a friend/patient, afro-american who takes no meds. He was on insulin but stopped it all on his own. He is 300+ pounds. He is getting a lot of plaque (white) build up only in the corners of his mouth that he actually washes away. He is using neosporin and abreve. Any idea what this may be?

I am sorry to hear about your friend.

The symptom you are describing sounds like oral thrush , also known as Candidiasis. It is fairly common is individuals who have diabetes, especially if it is not well controlled with the glucoses running persistently high. This could very likely be the case with your friend considering he has stopped treatment.

However, such a diagnosis can be confirmed only on proper in person examination and sometimes a quick test, in which the doctor may tale a swab sample and prepare a slide to assess under a microscope.

Further, there can be few other conditions too that cause such an appearance on the corners of the mouth and a dermatologist is the best person to evaluate for these.

Thrush is typically easy to treat with appropriate medications and of course the diabetes needs to be controlled too to prevent it from occurring again
