Question: Hello Doc,
I had one black rotten pigmentation on the sole of my right leg for the past 2 years but it is spreading more with similar smaller spots on right foot's sole. can you please tell me what it could be?

Hello. Thank you for writing to us.

I have gone through your query and I have also reviewed the Images.

These are plantar warts. It is an infection caused by Human Papilloma Virus (HPV).

I suggest that you either get them Freezed with Liquid Nitrogen (Cryotherapy) from a dermatologist.
Liquid nitrogen would freeze wart/infection bearing skin which would blister next day and peel off in about a week revealing normal skin.

Or

You may also use a topical keratolytic solution containing salicylic acid 17% + lactic acid 17%, over affected areas, once daily at night.
It is a prescription formulation and dermatologists commonly prescribe it for warts, corns and callouses.

This liquid would soften affected area and gradually peel them off.

Before using this liquid, soak your feet in lukewarm water for 15 minutes at night, towel dry and thereafter apply this liquid over the warty bearing area, let it dry and leave it overnight.

I suggest that you visit a dermatologist in your vicinity for the needful.

I have few questions:
1. the solution alone will suffice or it requires any other oral medication as well?
2. How many days it would take to Cure, if I use topical keratolytic solution?
3. How to permanently get rid of HPV? Will it be there in blood?

Hi

Solution can be used alone.
No Oral treatment is required though some dermatologist also ask patients to take Oral Zinc but this is not a standard treatment for warts.

The duration of treatment would vary depending on the number and how deep/ thick is the wart bearing skin.
Superficial warts would shed early and deeper would take time.
In general 4-6 weeks of treatment with keratolytic solution is accepted as adequate for treatment.

Cryotherapy would give faster results as compared to solution since it freezes the wart bearing tissue which blisters and peels off revealing new skin within a week

Treatment usually gets rid of infection permanently. The infection is limited to skin. There is no infection in blood.
