What Causes Persistent Vomiting, Weight Loss And Abdominal Pain?

Question: son in almost constant abdominal pain, has lost over 40 lbs. nothing diagnosed after 3 years, vomiting 4 to 5 times a week. can eat almost nothing. Has almost constant abdominal pain. Extreme pallor, dehydrated has extremely low energy.

to avoid asking irrelevant questions about his case, please upload any report you have right now (colonoscopy, CT scans, MRI scans, ultrasound scans, biochemical panel, complete blood count, inflammatory markers like CRP, ESR, etc). Please also provide as many clinical details as possible. For example:
- does it hurt all day or does it come and go? And if it comes and goes, what precipitates an attack or worsening and what comforts him (if anything)?
- does he have other associated symptoms like nausea, vomiting, etc or are these symptoms irrelevant to the pain?
- Does the pain spread to other locations of his body?
- Does going to the toilet or passing gas make any difference?
- What's the character of the pain (aching, squeezing, etc)
- Do you have a family history of unexplained abdominal pain or unexplained fever?
- any symptoms from his joints?
- his profession and any potential exposures to toxic substances like lead or others
- any rash? Tick bites? Regular contact with domestic or wild animals?
- anything to note from clinical examination (as his doctors may have informed you)?
- does he use any drugs? What drugs and why?
