Posted on Thu, 21 Apr 2016
Question: Hi,

For the last month I have had a tingling sensation in my right hand and arm usually first thing in the morning and tended to go away after a while. For the last few days it comes and goes also during the day. There were also days where it wasn't there at all. Do I need to be worried? I'm 32 years old and in good health, when going running or doing yoga I'm fine

Brief Answer:
More information needed

Detailed Answer:
Dear,
Welcome to HCM
There is no need to worry much, as the symptoms are not suggestive of any serious condition. However, one must investigate further for a specific underlying cause.
I would like to consider following differential conditions that are commonly known to cause such symptoms, such a neuralgia, radiculopathy, neuropathies etc.
To find a more specific condition, I will need to know about following-
1) Is there an associated pain? "Shooting type" pain? Muscle spasms?
2) The numbness and tingling, does it changes its site? Or it is been present over same area?
3) what aggravates the symptom ? And how it gets relieved?
4) any back pain problem?
5) present on which side?
6) lastly, any history of fever, injury?

The list of questions may appear bit exhaustive, but it is important to include all relevant information before one commits to a diagnosis.

Thanks so much for answering...
1) no pain as such. But it's like the arm is falling asleep and then starts tingling... A few times I think that sensation woke me first thing in the morning. But it keeps coming back throughout the day. No
Muscle spasm
2.) only ever present on the right side. Mainly the hand sometimes extended into the arm
3.) no idea what aggravets the symptom. Certainly when I lie on my right side and therefore put weight on my right hand and arm sit gets worse... When it happens I try to move my right hand and arm as much as i can. That helps but it goes away on its own but comes back also on its own
4.) no not at all
5.) right side only
6.) not at all

My biggest worry would be MS. Just by googling the sympto

Brief Answer:
an x Ray spine and vitamin b12 investigations are advisable

Detailed Answer:
Welcome back. Thanks for the reply.
From the information I could establish that there is a symptom of tingling and numbness, which is intermittent, no apparent aggravating factors and is confined to right arm, hand.
Regarding the possibility of MS, it has an insidious onset, tingling or numbness could be one symptom presenting, but an isolated symptom of such is not suggestive of MS.
MS can be suspected if there were associated complaints of fatigue, vision problem, weakness of limbs, tingling or numbness that is not just confined to one part or area, pain, dizziness, difficulty in controlling bladder, speech difficulty, etc.
Personally, I am suspecting a more common causes such as
nerve entrapments, brachial plexus neuralgia, nutritional deficiency related neuropathy, radiculopathy, etc.
Considering the history and information that we have, a clinical neurological examination is required to further examine the spine and elicit diagnostic signs.
My suggestions to you are
1) A consultation to local neurologist strongly is advisable, as he can perform tests and local neurological examinations. It is essential.
2) Investigations of Xray cervical spine, vitamin b12 levels are some preliminary investigations that are strongly advised.

Further comments about the cause can only be made after studying the cervical spine and knowing if there is any underlying nutritional deficiency. The management and treatment depends accordingly.
Meanwhile also keep a check on development of associated symptoms if any and if there is progression of the symptom.

Hope to have satisfied your query.
Welcome back for more information.
stay healthy.
