What Causes Persistent Shoulder Pain After A Shoulder Labral Tear?

Question: Recently I had an MRI of my shoulder for pain.
Supraspinatus undersurface fraying with pinhole full thickness tear at the junction of the supraspinatus and infraspinatus.
My ortho told me that I have a 75% chance of healing with PT.
I just started PT about 2 weeks ago and still having pain.

My concern: I just started a new job AND cannot even consider surgery.

I'm scared and worried
What are your thoughts?

A pinhole size full thickness tear is not something to be worried about. It will heal with proper rest and physiotherapy. Do continue with conservative treatment. The pain will gradually subside. There is no need to consider for surgery now.
Do take mild pain killers for symptomatic relief.

The MRI also mentioned that there is tendinopathy of the supraspinatus tendon, with undersurface fraying of the entire supraspinatus, beceps tendon intact.

There is a tear of the anterior superior and posteriour superior labrum - this does not seem to be causing pain - my ORTHO did not seem to be overly concerned about this.

Do these injuries really heal with physical therapy and rest?

Rest will help in healing.

Hello.
Thanks for writing again.
The tear of labrum is late to heal if it is a full thickness tear. Rest of the fraying and tendinopathy will heal with rest. Avoid all painful movements and do not overdo the physical therapy for an early healing.
You can avoid all overhead movements also till it is properly healed. Rest of 3 weeks will help.
