Question: GOOD MORNING,
MY DAUGHTER AGE 11YRS. JUST RECOVERED FROM DENGUE NS1 WEAK POSITIVE.
SIR IT HAS BEEN NOW 10-15 DAYS SINCE SHE RECOVERED WELL AND STILL SHE IS NOT GOING TO SCHOOL. BUT RECENTLY I HAVE COME ACROSS FEW TIMES PANIC ATTACKS IN HER WHICH LASTED FOR SOMETIME AND THEN SHE SLEPT AND THIS OCCURED WHILE SHE IS SLEEPING. ALSO SHE HAS A ACUTE CONSTIPATION WHICH SHE FEELS DURING THOSE ATTACKS.
SIR I JUST WANT TO KNOW THE EXACT REASON FOR THE SAME. SHE IS FULLY CONSCIOUS OF ALL THE THINGS DURING ATTACKS.

I have gone through your question and understand your concerns but don't worry. I will be able to give you the best possible advice only after knowing her symptoms clearly. So, kindly mention the symptoms and timings when she has the 'panic attacks'. How is she at school and are there any known stressors in school or family environment?

How severe is the constipation? What medications do you give for nausea, acidity and mesenteric lymphadenopathy? Also, tell me the duration of this treatment.

PANIC ATTACKS OCCURED AFTER 7-8 DAYS OF RECOVERY FROM DENGUE NS1.
THE FIRST ATTACK OCCURED WHEN SHE WOKE UP IN THE EVENING 6 DAYS AGO. BUT YESTERDAY NIGHT AROUND 11.30 PM THE SAME ATTACK OCCURED.
DURING THAT PERIOD SHE BEHAVES VERY DIFFERENTLY.
1. SHE IS FULLY AWARE OF WHAT IS HAPPENING AROUND HER BUT SHE DOESNT COME TO KNOW WHAT TO DO OR NOT. SHE RUNS HERE AND THERE AND TALKS IRRELEVANT THINGS. BUT SHE ALWAYS RUNS TO THE WASHROOM AS IF SHE IS TO PASS THE MOTION.

2. SHE IS HAVING ACUTE CONSTIPATION SINCE SHE GOT AFFECTED. EARLIER SHE NEVER HAD CONSTIPATION . SHE FORCE HERSELF THESE DAYS FOR STOMACH CLEARANCE BUT SOMETIMES GET RESTRICTED AND IS WORRIED ABOURT THE SAME.

3. SHE IS VERY GOOD IN SCHOOL AND SHE IS ONE OF THE TOPPERS. MAYBE SHE IS OFF FOR SCHOOL FOR THE LAST SO MANY DAYS AND EVEN SHE TALKS OF THAT THE CLASS MATES WILL TEASE HER FOR THE SAME AS SHE HAD 100% ATTENDANCE ALWAYS.

4. SHE ALWAYS TAKES PART IN ALL THE COMPETITIONS WHICH FOR THE TIME BEING NOT BEING ABLE TO.

5. MEDICINES: EMERSET
CYCLOPAM
ACILOC
ZANOCIN 200

SHE JUST HAD A STOMACH INFECTION WHICH OCCURED ON SUNDAY AFTER WE GOT ULTRASOUND DONE WITH CT CUT. REACTIVE MESENTRIC NODES WERE THERE FOR WHICH SHE IS TAKING MEDICICNE FOR THE LAST 5 DAYS.

OTHERWISE SHE IS VERY WEAK BOTH PHYSICALLY AND MENTALLY.
MAYBE SHE TAKES EVERYTHING VERY SERIOUSLY.

Start her on Ofloxacin- ornidazole combination drug

Detailed Answer:
Hi
I have gone through your description. She seems to be suffering from gastrointestinal infection leading to mesenteric lymphadenopathy. This can be best treated by Ofloxacin-ornodazole combination twice daily for 3 days. Give lactulose syrup for constipation and continue with aciloc, emeset and cyclopam as needed.

Her behaviour can be due to post febrile encephalopathy, seizure disorder or psychological illness. Anything definitive can be said only after complete neurological examination by neurologist and psychotherapist. They may recommend you for an EEG and MRI Brain if required. Once an organic cause is ruled out then psychological illness needs complete evaluation by psychotherapist for proper behavioural therapy and medication if needed.
