What Causes Painful Premolar Teeth?

Question: nullI have an odd problem in My premolar(Tooth#30,molar on bottom right side).It started with sensitivty to cold and hot.Later on I felt pain emitting out.I visited Dentist locally,they took bite-wing X-Ray and some others but did not show much.I have a composite tooth feeling in that premolar.Later on I felt pain in biting as well as the gum below the tooth wherein it connects to the tooth is in Pain.My dentist gave me painkillers and Amoxicillin 500 mg capsules.She also recommended scaling of the bottom right and right upper sections of the mouth.I have already completed those.After completing the course the pain and swelling has gone away but at the bottom of the molar in the gum area there is a hard lump like thing ,not visible externally,can only be felt by finger and painless unless I press it.If we look in mirror it does not show any visible swelling from outside.I am very worried.

It seems to be periodontal abscess & needs gingival curretage & deep scaling.
Also take metrogyl 200 twice a day & warm saline rinses.
If still problem persists it might require Root Canal Treatment.
Visit your dentist & get treatment started.

Deep Scaling is done already
Completed 500 Mg Amoxiciilin course as well.
I will do the saline rinses.This is going on for over 3 weeks now.I had my deep cleaning done last week.

I went to the Endodontist today for an evaluation.
He tapped my tooth using an instrument and noted negative percussion.
The endo ice test was positive,I did not feel anything in the affected tooth.
He recommends root canal followed by crown,what is your thought?

If endo test is positive then you should go for RCT followed by capping.
Some times percussion may be negative but your endo test & symptoms suggest go for RCT.

I have uploaded a new X Ray for your review.

An additional concern in the lump few cms below the tooth in the gum area.its really sore and hard to press.Also the gum is tender there.

It has developed abscess.

There is infection in that tooth.
You have already gone for scaling & curretage but problem still persists.
Now go for RCT followed by capping if you want to save tooth.
Give some time between RCT & capping.
