What Causes Painful Bowel Movement Along With Itching On The Buttocks?

Question: I am a 41 yr old woman. I am experiencing buttock itch and blood on toilet tissue on and off for several months. Bleeding is very small dots in the toilet paper and usually happens after wiping several times so I'm wondering if it's irritation from wiping. But in the past few days have been experience itching. Bowel movements are not painful and I am not constipated. What should I do?

Most likely you are having hemorrhoids, which are inflamed veins in the anus or rectum. Irritation to these veins can make you bleed itch and burn after and during bowel movements.

I would suggest you plan a visit to a general surgeon and get a proctoscopy and digital examination of rectum done to rule out hemorrhoids as a cause of the problem.

If hemorrhoids are diagnosed then conservative management would be started.

You can use Preparation-H, a combination containing contains Calcium Dobesilate, Hydrocortisone, Lignocaine, and Zinc for relief till the test is done.

Diet suggestion-

Consume high fiber foods as a part of high fiber diet.

Grain products: (6-11 servings of grain product in your diet is required daily) whole grain breads, whole wheat, whole grain rye, mixed grain, cracked wheat, buns, muffins. Brown rice whole wheat pasta or brown rice pasta cereals made from whole grain oatmeal ground flax, wheat germ or wheat XXXXXXX

In baked goods vegetables: (3-5 servings daily) dark green leafy vegetables contain highest amounts of fiber e.g. Spinach, broccoli, green peas, green beans, turnip greens.

You're not concerned about it being something more serious? I just assumed for months it was skin irritation and then when the itching started I looked it up and the first thing to come up was anal cancer .

Thats why i asked you to go for proctoscopy if its normal then we have to go for colonoscopy.
My dear friend thumb rule of medical sciences is common thing first.
We start from most common thing and move toward complicated things.

As per your narration-
Bleeding is usually there while wiping (Tell tale hemorrhoid)
Itching is there
No pain.
Age 41.
Very likely to be hemorrhoid.
Symptoms of anal cancer are usually more aggressive and patient has profuse bleed with severe pain.

So have faith on me.I am just following the protocol.
If proctoscopy is normal then we would definitely proceed with sigmoidoscopy and colonoscopy.



As per standard literature-
When a patient presents with bleed after the age of 50, it is recommended that he/she should have a colonoscopy initially but as you are just 41 intially go for proctoscopy.
