Question: I wake up with a pain under my right shoulder blade. Feels muscular but keeps me awake. I needed 2 ib. to get back to sleep last night. It has happened 2 nights in a row. Might be tingly in right arm but very slight. I am 78 female no history of heart disease. I have low pulse and BP from years of running and exercise. No V or sweating. I am on no meds. Did have breast cancer Stage 1 11 years ago. I have had dizzy episodes on and off for years starting 20 years ago with real spinning.

Brief Answer:
Probably musculo-skeletic pain

Detailed Answer:
Greetings and welcome to HCM. I understand your concern. Thank you for your question.

This kind of pain, located only in one position, that starts after being asleep, probably refers to a muscular or bone origin of the pain. I think a five-day course with ibuprofen three times a day would help and is recommended. However, given your age and history of breast cancer eleven years ago, I would recommend some tests, just to be "in the clear":
- a chest and neck spine X-ray - to confirm or exclude any poasible origin of the pain in the area;
- a complete blood count
- a complete biochemical panel, including urea and creatinine, blood glucose levels, liver function tests and electrolyte count.

I hope this helpful. The above mentioned tests are recommended if the pain persists after the course with ibuprofen. I am happy to help, if you have follow-up questions.

Best regards,
Dr. Meriton
