What Causes Pain In Testicle Relieved With Masturbation?

Question: Hello Doctor,
I am 24 years old, male.
Sometimes, i get a gradually increasing pain in my testicle, that goes away after masturbating.
What is the reason for this pain.?
I have also observed that i don't get this pain if i masturbate at least two times a week.
Pl explain this problem

I can understand your concern.

Pain relieved with masturbation suggests possibility of Spermatocele (fluid buildup in the testicle ).This spermatocele might not be totally closed. When there is masturbation sperms ejaculated from this Spermatocele and no pain.. But when masturbation is postponed for long duration sperms get accumulated in Spermatocele and causing pain.This possibility is suggested from history you mentioned.

There are many other causes of pain testicles.

I need more information to guide you further.

- Is pain in single or both testicles?Since when is it there?Do you feel any lump/swelling in scrotum?

- Any associated symptoms like swelling, redness,tenderness of the testicle
and/or scrotum, nausea, vomiting, fever, abdominal pain , burning in urine or
urination difficulties are present there?

- Any other physical problem present?

- Have you done sex ever? Is there any discharge from penis?

- Is there any injury to testis in past?

-Pain is mostly concentrated on one side.
-have observed slight swelling in testicles during pain. No other symptoms that you have mentioned were present.
-Yes. I have had sex. I haven't observed any abnormal discharge from penis.
- No injuries in the past

You forgot to mention since when pain is there.

However above information conclude that no major problem is there. It is likely spermatocele. Normally it does not require treatment. However if pain is there and troubling it can be removed surgically.

I suggest you to visit general surgeon. Avoid masturbation for few days. When you feel pain, go to doctor. At that time examination and if needed ultrasound should be done. It will help to confirm diagnosis.

I have observed this pain for the past six years doctor.
Is there anything called Blue Balls???
I browsed online and found that this problem is also called as blue balls. Is it so ??
Does surgically removing it affect my fertility in any ways???

Blue balls pain of testis: Sexual arousal that does not terminate in ejaculation can cause congestion in the sperm carrying tubes, resulting in a dull aching sensation in the testicle.
This is sometimes referred to as ‘blue balls’, which is inconvenient but entirely harmless.
The pain disappears within a few hours anyway, although some men relieve the discomfort through masturbation.


This happen when you avoid masturbation for long. You mentioned that you are dong masturbation so less chances. By chance if this diagnosis in your case is there , no need of any surgery. Just do regular masturbation. Complete your sexual desire or arousal with ejaculation.

And if it is spermatocele , surgery of the same is safe and does not affect future sex life or fertility.
