Brief Answer:
needs infestigation to rule out systemic causes

Detailed Answer:
Hi,

I went through the query and understood your concern.

The severe pain below knee joint in morning hour with no other local problem (as examined by orthopedic surgeon and vascular surgeon ) may be due to any of the following
1. Low electrolyte levels. Potassium or calcium. Get checked for the serum calcium and potassium levels.
2. Low protein with heavy activities. This needs supplementation of protein and protein rich diet.
3. Abnormal posture or tight dress during sleep which may affect blood supply to the lower limb and hence severe pain in morning hours.
4. Any recent local trauma.
Considering your age get checked for blood glucose and fasting lipid profile once to rule out any problem in them.

So in summary get checked for your electrolyte levels along with RA factor, ESR level (to rule out Rheumatoid arthritis) and for now take analgesic drugs like ibuprofen for pain.

I hope this information helps you. Feel free to ask for any further queries. I will be glad to answer them.

Wish you good health.
Regards
