Question: Hello,
I'm a 50 yr old male and just in the last week I am having some really bad rectal pain. It is a constant hurt, there is no pressure that I feel, only pain. When I cough or sneeze, the pain is really bad, sitting even hurts. I have no pain when passing stools but have noticed a slight decrease in number of times to go. Nothing is hanging out or bulging, just this constant pain.
Can you maybe direct me to the issue and treatment?

The most common reason for rectal pain are hemorrhoids or rectal fissure. There might be no visible lumps. You may notice some bright red blood on the stool surface.
As the prostate is attached to the rectal wall, prostate inflammation can be the case as well. If so, the pain can radiate to the scrotum. Burning while urinating can be present.

As you are 50, it is time for your first colonoscopy. Rectal cancer usually does not hurt, so it is not it. But still you need this test.

Here is what you should do:
- consult general surgeon and have rectal examination
- in a meanwhile try hemorrhoids suppositories
- check your PSA level (for prostate issues)
- get scheduled for colonoscopy
