Brief Answer:
Urethritis

Detailed Answer:
Hi.
Pain in penis only on urination and ejaculation is suggestive of Urethritis; less likely to be yeast infection. This can be a part of UTI.

I hope by this time you must have through the relevant blood tests, urine- routine, microscopy, culture and sensitivity reports; ultrasonography of the abdomen particularly for urinary system.

If not , get this done after a consultation of an Urologist; Urethroscopy and cystoscopy give one the diagnosis. The treatment with antibiotics, alkalinizer will help to solve the problem.
Till then have plenty of water orally.
If you are sexually active use condom and get your partner examined and treated simultaneously.

I hope this answer helps you to go into a proper way of diagnosis and treatment.
