Question: lasertripsy surgery 23 july. Pain in lower back and left leg to knee ever since. epidural injections , PT of no help. MRI shows no significant problem(arthritis) for my age(80) Walking/standing for 10-15 min causes discomfort in lower back All pain on left side No back or hip problems prior to kidney procedure

I am Dr. Prakash HM and I will be answering your query.
Based on your query, my opinion is as follows:
1. Possibly not related to lithotripsy. It should be mild pain and around procedure period only. So late, cannot be lithotripsy cause. If it is, taking a dicyclomine or antispasmodics should help. I doubt, it would.
2. Could be due to weak core muscles of abdomen and back. More in the back.
3. Will need active exercises for strengthening muscles. Epidural injections are for neural cause and here it does not appear to be, even though radiation in the form of sciatica is seen. As MRI scan is normal, no nerve compression or vertebral column cause.
4. Muscles weakness are only possibility, and active exercises rather than passive physiotherapy would be helpful. Also lumbar belts along with walking stick for better weight transfer and support would be helpful.
Have good hydration and protein rich diet. Hope you are not overweight.

latest opinion suggests nerve compression due to slight misalignment at L4--definitely nerve related' Neuro Surgeon. Core strength very good--5-10 lb over ideal weight. I am trying to avoid back surgery.

Thanks for replying.

Based on report of MRI being normal, nerve related cause was ruled out.

However, nerve compression is not good. Surgery is difficult at this age, but if necessary, can be done. Altering your lifestyle, lumbar spondylosis can be managed easily.

Other options include:

1. Core strengthening, specifically of back muscles. To avoid further pressure on vertebrae, so to avoid further compression of nerve. As your core strength is good, try to continue exercises to maintain it the same.
2. You can use lumbar support braces
3. Limit work to maintaining personal activities. Avoid any sort of weight bearing exercises, which can put pressure on the vertebrae.
4. Maintaining proper posture.
5. Using a good lumbar support bed
6. Painkillers during time of severe pain.
7. Alternate physiotherapy to relieve pain can be taken.
8. Steroid injections near irritated nerve should help during periods of severe pain.
9. If overweight, will need weight loss.
