Brief Answer:
Diagnostic is necessary to determine the reason.

Detailed Answer:
Hello!

Thank you for the query.

Abdominal pain and back pain can appear due to several issues including kidney infection or stones (even if you have no problems with urinating), gallstones, uterine and ovaries issues or even spine disc problem.

Kidney infection or stones tend to give back pain radiating to groin. If both kidneys are affected, the pain can radiate to both groins. Nausea and fever can be present with this issue.

Gallstones can give upper middle and right abdominal pain which can radiate to the back. Nausea and vomiting can appear. Fatty foods can increase the pain.

Spine disc pain can give similar to described by you pain. However should not give nausea and should get more intense after physical activity.

I suggest you to have some tests done. Abdominal ultrasound, blood work, urine analysis should be done. You should also consult gynecologist.

Hope this will help. Feel free to ask further questions.
Regards.

Brief Answer:
Abdominal ultrasound should be done.

Detailed Answer:
This can point to gallstones. Stomach diseases (like an ulcer) do not cause back radiation. So please have abdominal ultrasound done.
