Based on history you shared, the fever and pain below lungs rise possibility of many things, as below lung in left the stomach pain, spleen and pancreas are all possible. It would have been more appreciable if we had known about any frequency of cough, or associated signs of respiratory tract infection.

You may need to undergo, chest Xray and abdominal utrasound along with detailed examination of the site of the pain to rule out all these possibilities.

The presence of polycyclic kidney disease, may have arouse the possibility of ruptured cyst, only in setting of bleeding in urine, but no such history males it less likely.

In any case, you need to promptly consult nearby health facility to work out all the possibilities.

This BP with the angiotensin receptor blocking medicine which you are already using, needs further evaluation and adding of drugs as with kidney diseases target should be 130/90.

The fever need blood screening with complete blood counts, CRP, and ESR along with renal function disease for your kidney issue as it can rise in setting of acute infection.

Meanwhile you can use good analgesic and start antibiotic after review with your GP.
