Brief Answer:
It may due to neck muscle sprain or spasm.

Detailed Answer:

Hello,

As per your history, he doesn't have an earache, but pain behind the ear along with pain in the left arm, I think its most probably due neck muscle sprain or spasm which may occur because of improper posture, improper sleeping position, cervical spondylosis, arthritis, injury to the neck.

Other causes of pain behind the ear is mastoiditis, referral pain due to dental caries. Kindly follow with detailed history.

If tab. Tylenol is ineffective, you can take tab. Metaxalone and diclofenac. Apply Diclofenac gel or spray locally. Do physiotherapy. Don't give much pressure neck. If it's because of spondylosis, then he may need neck traction.

You can do x-ray or MRI neck to rule out other conditions. Try to maintain proper posture while sitting and sleeping.

I think this will clarify your concerns.

Thank you.

Take care.
