What Causes Pain And Lumps In Stomach?

default
Posted on Wed, 8 Oct 2014
Question: HI my mum is 65 years of age and has had severe pain in her stomach area and she can feel lumps in the area. for while, she has had blood tests and liver function, kidney function blood etc have all come back normal, However she has an ESR of 52, and has also had a ct scan and has a build up of fluid in her endometrial cavity. she is waiting for a referral to Gynae, and her Gp is suggesting she may have IBS is there anything else I can ask for her to be tested for? this has been going on for a while.

I forgot to add that the CT scan showed infection in her intestines

I also had a TB test done on my mum, An interferon Gamma release assay. this test came back negative. however in extra pulmonary disease the sensitivity of this is around 90% so i am not sure I can rule this out.

Brief Answer:
Rule out organic issues first before saying IBS

Detailed Answer:
Hi XXXX

I am sorry for the situation your mum is in.

The diagnosis of IBS is confirmed after excluding other organic pathologies. If there is no changes in the bowel movement, then, we cannot think over IBS or intestinal problems.

At this point, I'd like to know if:
- she is having problems with bowel movements: constipation ? diarrhea?
- is there blood when she passes the stool? any mucus?
- has she had any upper endoscopy to rule out GERD/gastritis? Often, the pain in her stomach area might be related to GERD/gastritis. Upper endoscopy is recommended at this case. Problems with her genital tract should be also excluded by gynecologist.

Having a high ESR reading is indicating that there is inflammation inside her body. Further investigations are needed to catch the source of inflammation.

The presence of liquid around her genital organs, then, this is indicative for possible genital inflammation or problems. Can wait until she meets her gynecologist for further evaluation.

You can send a copy of ct-scan report, so I can analyze it further myself.

If the quantity of liquid is large, a part of it can be sucked to run biopsy.

Hope it helped!
