Brief Answer:
Ischemic heart disease needs to be ruled out first.

Detailed Answer:
Hi,

There are quite a few possibilities in your case unless you give a detailed description.

GERD is one of the common reasons, alcohol and smokers have it often.

Similarly asthmatics commonly complain of similar symptoms.

Heart disease should be ruled out first, as it is one of the more serious causes for chest tightness, with this in mind, I would suggest you to get yourself evaluated at the earliest to rule out heart diseases/myocardial infarction or ischemia.

You can add prilosec as well to the list of medications you take.

Please write back with a detailed description of problem with duration, what aggravates and what relieves the tightness so that I can guide you further.

Best wishes.
