Question: I've had a stomach ache that comes and goes within a day for the last 2-3 months. the first time it came on was the worst, and since then when it comes it is less painful but definitely uncomfortable. It started around the time I began eating activia and I started using a Neti pot for bad sinus congestion. it seems to coincide with drinking coffee creamer, but not every time I have a coffee with regular creamer

To recapitulate:in a nutshell :
32 / M - nexium made bloating - Zantac works - your friend thinks this is an ulcer - C/O > suffering for 3 months - stomach ache that comes and goes within a day - coincides with coffee creamer but not with every time
you have a coffee with regular creamer...- started around the time you began eating activia and started Neti -

I would like to have a further information for me to help you more:
-The site of pain ? - Whether upper abdomen or any specific area or all over the abdomen ?
-Relation with food - does the pain increase or reduce after food? if yes, withing how much time ?
-Character of pain : Is it burning / twisting / throbbing or so ?
-How are the stools ? normal / black / well formed / loose/ hard ?
-Any pain in the central chest area...
-Any other problems like bloating or anything which you may think to be related or unrelated to the present problem.?
-Any investigations done for sinusitis ?
Please provide me more information...

As per the history of Nexium (Esomeprzole) / Zantac (Ranitidine), coincidence with Neti and Activia (yogurt) / Coffee creamer, the diagnosis goes more in favor of Gastritis with / without intolerance to some food / beverages.

It is known that anything can cause any problem any time .

I would advise in such a case the following with reasons:
Stop Neti and Activia (Yogurt) and coffee creamer.
This is to make sure whether anything is causing the problem you are having.
Once the problem is settled, each one can be started - one by one and to see whether a particular thing is causing a problem. This is important as the basic rule of any treatment is to identify and stop the allergen...

Continue Zantac twice daily.
Antacid liquid 3 tsf when on empty stomach.
Soft bland diet, small quantities at more times - at least divided in 4 parts.

See the results.
If the problem is gone, relax and do not re-start the products you have noted to increase / cause your problems.

If the problem persists, get an Upper GI endoscopy done. Continue the treatment as per the findings and the advise of a Gastroenterologist.

If there are associated symptoms :
Get blood, urine and stool tests and ultrasonography of the abdomen done.

ok yeah I left that out. the pain is all over. it's not sharp, just there. can almost feel like a burning. Sometimes it seems to start when I'm sitting hunched over, but the other day it started while I was walking my dog and within a few minutes of drinking a coffee with cream(if I drink coffee with non-fat creamer then I'm fine) sometimes it seems to start on an empty stomach but sometimes it seems to start within an hour of eating. I should note that in the 3 months since the first stomach ache I would count the number of reoccurrences at 3-6. stool is hard because I'm on a methadone program for past addiction to opiates. it's been almost 3 years and I'm planning on tapering off after next fall when I graduate college and start looking for a career(that is why I was eating activia in the first place). I was diagnosed with acid reflux like 10 years ago but that was probably caused by heavy drinking at the time(I no longer drink alcohol)

Read it carefully and understood..

This is very clear that this may be a an acid peptic disease and be confirmed by Upper GI Endoscopy and treated accordingly,
stop the instigating factors as you have already done,
Laxative for the hard stools as they can add-on the problems.

Rest of the things to be followed as already explained above, you can certainly discuss this with the Methodone Program in-charge, if it has anything to do with the present problem. Well, activia looks to be a contributory factor. Please ask for the best laxative your Doctor suggests in such cases.
