Question: I am a Realtor by profession in a resort market and so 90% of my clients live away from my market. I type a great deal on my keyboard and work, and then have been working on my keyboard at home for years on a novel. I use only thumb and forefingers. Feeling numbness on surface - very noticeable. But also feel the numbness in my other fingertips. My hand starts to hurt if I am holding anything up for a period of time. Or, if at self car wash, cant hold wand in my hand very long before have to switch to other hand because hand starts to ache. Feel slight discomfort in hands. All this started in last few months.

You are concerned about having tingling in the 1st and second fingers along with having pain in the wrist and hands with holding a wand in the car wash. These symptoms may indicate a compression of the median nerve in your hand from years of typing. This is very common problem in our society today and is known as carpal tunnel syndrome. The median nerve in the wrist gets compressed from over repetitive work like typing. This nerve gives sensation in the thumb and index and third finger. This tingling can happen anytime of the day but seems to happen frequently at night. Pain is also associate with the compression usually again from holding an object. From your description I think you may well have carpal tunnel syndrome.

For treatment I recommend starting and anti-inflammatory like ibuprofen, also wearing a carpal tunnel wrist. I also recommend trying a split keyboard which may help reduce the stress on your wrist and nerves. Give these measures a few weeks and if they do not help then you should seek medical attention.

Your doctor may have you continue the above measures and start you on physical therapy. They may also offer steroid injections to reduce the swelling and pressure. Lastly they may need to do surgery to relieve the overall pressure in the median nerve.
