Brief Answer:
hi and welcome

Detailed Answer:
It can come back but this is still not sign that it is and you should wait for few more days and see what happens. Usually even after antibiotic course, symptoms persist for several days and this is not strange. You should avoid cold bevearages, rest, use anticongestants and take vitamin c. If symptoms persist for 2,3 days you need to have swab of throat and verify exact pathogen so that right antibiotic can be used.Wish your good health. Regards

Thank you for that information. But, I want to be sure that I communicate correctly regarding my symptoms. By last Friday, I was feeling much better, no fever, no other symptoms except my throat/tonsils still felt a bit large. The symptoms I have now - 100 degree temp, sort of a swollen feeling in throat, and some pressure at the top of my nose - just showed up again last night. So, if I understand you correctly, you are saying that the symptoms can go away almost completely and then return again as they have today, and that is not an uncommon occurrence?. And, that I should wait to see if symptoms get worse, and if they do, go back to doc's office?

Brief Answer:
hi

Detailed Answer:
Yes that's right. 10 days of antibiotic course is enough and it would not be beneficial to start penicillin again at this point. After strep infection, our body is weakened and more sensitive to other bacterial and especially viral infection so it is possible to be free of symptoms for few days which could occur again but in low intensity. If this is viral infection now, then antibiotic won't be efficient and it should pass spontaneously in several days. If this is bacterial infection again then this will last for more days and this is probably caused by another bacteria or by more resistant type of strep and because of it you should do throat swab test and antibiogram to find exact type of pathogen and efficient antibiotic. Other possible causes of fever can be autoimmune disorders or rheumatic fever which are rare today but if rash, chest pain or joint pain occur then this should be considered and it must be treated in hospital
