Question: I have an anal itch that can be extreme at times. I get occasional discomfort with bowel movement. Occasionally there is some blood present on toilet paper/wipe. There has been zero blood in my stool.

I appreciate your concern

In such a presentation of symptoms the likely cause is pinworm infection or hemorrhoids. But malignancy should always be ruled out. Local examination should be done for any hemorrhoids, recurrence of fissures, fistula and other causes. Tape test overnight can also be done. General inspection , DRE ( Digital Rectal Exam ) and proctosigmoidoscopic examination could also be advised if indicated based on the physical exam. Stool routine examination for ova and parasite should also be done in consult with a proctologist. Treatment would depend upon the cause.
