Question: Dear Doctor, I have itching burning and irritation around anal. It is intensified after bowel. Is it piles. Kindly help me.

I have read your query,

Itching and burning around anus could be related to anal sweating or infection of intestinal parasites or infective diarrhea or candidiasis.

Itching/burning around anus due to piles is uncommon but should also be ruled out.

So,I would suggest you to see a doctor (preferably surgeon) who may examine your anal area (by visual inspection and digital examination) or by proctorsope to rule out piles.

He may also advise some routine blood counts, stool examination for R/M ova and cyst to rule out parasite infestations.

Meanwhile,you can apply any over the counter antihistamine cream (like benadryl) with lignocaine cream (xylocaine cream) for symptomatic relief from itching and burning after every bowel motion.

Oral tablet like cetrizine 10 mg twice daily can also be helpful if itching is severe.

I would also suggest you to take high fibers in diet (salad) like cucumber, cabbage, carrot etc.

Avoid spicy and oily foods. Avoid excess sugar in diet or tea or coffee. Take only soft diet in initial couple of days.

Avoid citrus fruits as they might cause further anal irritation.

So,consult surgeon for further evaluation.

Dear doctor,
I had visited the surgeon. He inspected the affected area. He found the ulcers on it . he had prescribed some antibiotics and a creme to apply for 5 days. After that he would be able to diagnose further. Besides this I want to consult about having acidity. I have acidity since 10-12 years. I am taking aciloc 150 mg twice daily for 10 years. Will it cause some complications in future? Is this itching problem is the result of it? What should I do to get rid of this problem? I am also taking calaptin 120 sr for palpitation. Kindly advice me. Thanks.

Welcome again,

Thanks for sharing information,

So you can continue with antibiotics and that cream at the anus where there are ulcers by your doctor's examination.

Aciloc 150 mg (ranitidine) is one of the SAFEST drug nowadays and probably there will not be having any problem taking it longer but it would be better to take it under supervision if you have to take it for long run though it is available over the counter.

There will not be having any complications in future. Your ans itching is highly unlikely related to this antacid,so no need to worry about it.

Acidity or stomach burning could be related to plenty of factors like excess stomach acid or acute gastritis,gastric ulcer or Helicobacter pylori infection or food allergy or stress or drug induced but should be further investigated as you are suffering from it from 10-12 years.

Calaptin SR (verapamil) may also cause stomach burning or acidity sometimes,you check it with your treating physician.

Meanwhile,you may take symptomatic treatment with STRICT dietary precautions.

Take proton pump inhibitors (pantoprazole 40 or rabeprazole 20) twice daily which will help to reduce excess acid production in stomach.if your Aciloc is not giving much relief.

You can also take digene gel or liquid or ulcero-protective agents (like sucralfate or bismuth subsallicylate) to reduce the burning sensation.

Apart from medications you should be careful with your diet.

1) Eat plain foods like boiled rice and green leafy vegetables without adding spices or oil. It will help to neutralize digestive acids and also they are easily digestible means there will be no need of extra stomach acid.

2) Avoid oily,spicy and fast foods

3) Avoid eating full stomach at a time, rather have small meals frequently.

4) Drink coconut water will help to reduce burning.

5) Eat lots of fruits like banana and watermelon.

6) Avoid excess tea and coffee if you take.

7) Eat or drink buttermilk and yogurt twice daily.

8) Chew ginger which releases stomach gases and thus helping in acidity and gas trouble.

9) Do morning exercises like walking or jogging which will reduce your stress level.

However,if the symptom is troublesome even after taking Aciloc and dietary restrictions,I would suggest you to see gastroenterologist for detailed medical history and further clinical to diagnose the cause.

Thanks doctor to advise me for my queries. I would like to know about one more problem which I often face. I have severe headaches. I feel havy weight on my head. Some times it lasts in 2-3 days. On that time there is heaviness in eyes and feel like morning sickness. It is sure while travelling. If it is intolerable , I take pain killer like combiflame. I think it is migrain. Is it so? Is migrain curable? Kindly help. Thanks.

Hello,
Thanks for asking again,

From all these facts that you shared here, it is more likely to be having tension type of headache with motion sickness occurs during traveling (not to be confused with a morning sickness which occurs during pregnancy).

Also it is usually relieved by taking over the counter appropriate pain killers (like combiflam which you take).

Whereas,migraine is a recurrent headache associated with nausea,vomiting or photo-phobia (extreme sensitivity to light). Generally it occurs at only one side of head and characterized by pulsating over head and mostly NOT relieved by usual over the counter pain killers.

So,I would keep first possibility of having a tension type of headache in your case as per your symptoms and descriptions. (As it is not accompanied by nausea or vomiting or other symptoms in your case).

And I will not think about this headache due to Migraine at present.

Tension headache may last for few hours to some days (may be 2 to 3 days or sometimes more) occasionally depending on triggering or precipitating factor.

Therefore, in my advice, you should continue to take alternative pain killer like Tab.Combifalm if it suits you on SOS (as and when required) basis.

You should also take tab.Stugerone forte 75mg before commencing journey or traveling to prevent motion sickness symptoms.

Anti-anxiety medication (like Recita forte) would also be helpful to reduce stress level and if you have not adequate sleep but you will need prescription from some local doctor or GP for it.

Take antacid which suits you like Pantoprazole 40 mg twice daily if you stomach upset.

Take plenty of fluids (simple water or lemon water) with lots of fruit juices (apple juice and sapota milkshake would be best). It would also be important to maintain proper hydration and electrolyte balance which in turn will help to reduce headache.
