What Causes Irregular Spotting Along With Pulling Sensation In Lower Abdomen?

default
Posted on Wed, 19 Aug 2015
Question: Hello! I had me tubes tied in 2007. I usually have regular periods that come in bold and strong. They usually last for 8 days and I only spot on the last day. For the past two months, I have had irregular periods. In XXXXXXX I spotted for a day before my cycle fully came in. It lasted for two days and then I spotted for two more days. This month (July) I spotted the day before my cycle came in and I only bled for one day. The blood flow was lighter than normal. I am now on day three of what should be my period, but I only see blood when I wipe myself. The blood is light pink in color. I also cramp heavy during my periods, but this month I only cramped on the day I was spotting. Other than that, I feel a "pulling" sensation on the right side of my lower abdomen. I also got a sharp pain that shot through my vagina yesterday. In addition to all of this, I have been having breast tenderness, bumps on my nipples, bouts of nausea, loss of appetite and extreme fatigue. I palpated my abdomen to try and get rid of the feeling of being too full (I thought maybe I had gas bubbles in there), and in doing so, I noticed that I have a knot about the size of a walnut in the lower right quadrant of my abdomen, about two inches above my pubic area, but under the belly button. What could be happening with my body?

Brief Answer:
Needs proper investigations.

Detailed Answer:
Hello,
Thanks for trusting us with your health concern.
You seem to have hormonal imbalance.
Please consult a gynecologist and have evaluation along the following lines -
1. Pelvic transvaginal ultrasound to evaluate the uterus and ovaries.
2. Hormonal profile - FSH, LH, TSH, Serum prolactin, adrenal hormones.
3. Complete blood count, liver and renal profile tests, glucose tolerance test.
Common causes of irregular cycles with lesser bleeding are - anxiety, stress, emotional turbulence, hectic travelling, recent changes in weight, ingestion of hormonal medications or medications that alter the menstrual cycle etc.
Polycystic ovaries, hypothyroidism and endometrial pathologies also could cause such a picture.
Sometimes, premature ovarian ageing could lead to pseudo menopausal symptoms, such as those you are experiencing.
The above suggested workup would point towards a specific cause for your problems.
Meanwhile, make sure you have an ideal BMI ( exercise regularly and eat healthy ), and that you are stress free.
The small knot in the right lower abdomen is likely to be a subcutaneous swelling - can be due to a collection of fat cells- lipoma. A direct examination is necessary to find the cause.
Please feel free to discuss further. I will be glad to answer the follow up queries that you have.
Wishing you good health.

Thank you for answering my question! I was wondering if breast growth could also be a sign of a hormonal imbalance. My breast are getting fuller and they have sort of a throbbing feeling. I did a self exam and there are no knots in my breasts.

Brief Answer:
It is a sign of hormonal imbalance.

Detailed Answer:
Hello.
Thanks for writing again.
The feeling of fullnes of the breast is related to hormonal changes in the body. Since there are no knots palpable there is no need to worry. Once the hormonal imbalance becomes normal the throbbing feeling will also decrease.
Hope my answer is helpful.
