Brief Answer:
It can be due to many reasons

Detailed Answer:
Hi
Welcome to the HCM

I understand your concerns but don't worry. The likely cause of such symptoms can be gastritis, arrhythmias, airway spasms due to hyperreactivity, hyperthyroidism, anemia or functional disorder.

I would recommend you to start him on Lansoprazole once before breakfast and go for an ECG( to rule out arrhythmias), chest X Ray and pulmonary function test (for airway condition). Also, rule out anemia and thyroid dysfunction by CBC and Thyroid function test. If normal and no other significant finding on clinical examination, visit a psychotherapist to rule out any functional disorders.

You may upload the reports for my reference so that I may help you out in the best possible way.

Hopefully it will be of help. I would be happy to help you in any further questions.
Take care
